开窗减压术和囊肿刮治术治疗青少年颌骨囊肿的效果比较

摘　　要：目的:比较开窗减压术和囊肿刮治术治疗青少年颌骨囊肿的效果,指导青少年颌骨囊肿的手术方式选择。方法:纳入2015年5月-2018年5月在首都医科大学大兴教学医院就诊的70例青少年颌骨囊肿患者,根据手术方案设定为两组,34例开窗减压术患者设为试验组,其余36例囊肿刮治术患者作为对照组。对比两组临床疗效、手术并发症(神经症状、术后感染)及对生活质量的影响。结果:两组术前及术后90、180、360 d的囊腔愈合速度、生活质量比较,差异均无统计学意义(P>0.05),试验组术后180 d的牙髓活力测量值明显优于对照组(P<0.05),试验组并发症发生率明显低于对照组(P<0.01)。结论:开窗减压术方案是患有颌骨囊肿青少年的首选治疗方案,有利于囊腔波及牙齿的牙髓活力的保留,减轻术后并发症。对依从性较好的患者,可优先选择开窗减压术。Objective:To compare the efficacy of fenestration decompression and cyst curettage in the treatment of juvenile jaw cyst,and to guide the selection of surgical methods for juvenile jaw cyst.Method:A total of 70 adolescent patients with jaw cyst from May 2015 to May 2018 accepted at Daxing Hospital Affiliated to Capital Medical University were included.According to the operation scheme,they were divide into two groups,34 patients with fenestration decompression were set as the experimental group and the other 36 patients with cyst curettage were set as the control group.The two groups’clinical effects,the surgical complications (neurological symptoms and postoperative infection) and the quality of life were compared.Result:There were no significant differences in the healing speed and quality of life between the two groups before operation and 90,180,360 days after operation (P>0.05).The dental pulp vitality of the experimental group at 180 days after operation was significantly better than that of the control group (P<0.05).The incidence of complications in the experimental group was significantly lower than that of the control group (P<0.01).Conclusion:Fenestration decompression is the first choice in the treatment of juvenile jaw cyst.It is conducive to the preservation of pulp vitality of teeth which affect by the cyst cavity,and reduce postoperative complications.For patients with good compliance,fenestration decompression can be preferred.
