摘 要:目的分析桂西地区壮族非小细胞肺癌(NSCLC)患者表皮生长因子受体(EGFR)基因突变与临床病理特征的相关性。方法以右江民族医学院附属医院2018年1月至2021年3月收集的NSCLC标本269例为研究对象,采用突变扩增系统聚合酶链反应(ARMS-PCR)法检测EGFR基因18~21号外显子的突变情况,分析基因突变情况与临床病理特征的相关性并做生存分析。结果269例桂西地区壮族NSCLC患者中EGFR基因总突变率为40.5%(109/269),其中女性突变率高于男性(51.4%vs.33.5%),腺癌突变率高于肺鳞癌(45.5%vs.8.3%),差异有统计学意义(P<0.05)。常见突变位点为19外显子19-Del,占总突变的55.0%(60/109);21外显子21-L858R,占总突变的29.4%(32/109);Logistic回归分析显示,性别、组织分型是EGFR突变的影响因素(P<0.05),与其他临床病理因素无关。EGFR突变型的肺癌患者经靶向治疗后,总生存率高于EGFR野生型(P<0.001)。结论桂西地区壮族NSCLC的女性、肺腺癌患者EGFR基因总突变率较高,而性别、组织分型是EGFR基因突变的独立影响因素。Objective To investigate the correlation between epidermal growth factor receptor(EGFR)gene mutation and clinicopathological features of Zhuang population in Guixi area with non-small cell lung cancer(NSCLC).Methods Two hundreds and sixty-nine cases of tissue in patients with NSCLC were collected from the Affiliated Hospital of Youjiang Medical University for Nationalities from January 2018 to March 2021.The mutations in EGFR gene 18 to 21 exons were detected by ARMS-PCR technology,and the correlation of clinicopathological features of patients with mutation status of EGFR and survival analysis were analyzed.Results The total mutation rate of EGFR gene was 40.5%(109/269)in 269 cases with NSCLC in Zhuang population in Guixi Area,among which the mutation rate of female were higher than male(51.4%vs.33.5%),the mutation rate of adenocarcinoma were higher than non-adenocarcinoma(45.5%vs.8.3%),differences were statistically significant(P<0.05).The common mutation sites were exon 1919-Del,and mutation rate was 55.0%(60/109).And exon 2121-L858R mutation rate was 29.4%(32/109).Logistic analysis showed that gender and tissue types were influenced factors of EGFR mutation(P<0.05).There were no statistical difference between other clinicopathologic factors.After targeted therapy,NSCLC patients with EGFR mutations had a better overall survival than non-mutated patients(P<0.001).Conclusion The total mutation rate of EGFR gene NSCLC patients of Zhuang population in Guixi area has been higher in female and lung adenocarcinoma patients,and gender and tissue types are independent factors on EGFR gene mutation.
