基于3D T_(1)WI序列的影像组学模型对儿童FCD/MCD的预测

摘  要:目的:探索基于3D T_(1)WI序列的影像组学模型对儿童局灶性皮质发育不良(FCD)和皮质发育畸形(MCD)病灶的预测效能。方法:回顾性搜集2015年3月至2020年8月难治性癫痫患者术前MR影像和临床数据。共获得46位患者的图像用于影像组学模型训练,男28例,女18例,平均年龄7.2岁,手术病理结果为FCD的39例(39/46),MCD 7例(7/46);其中FCDⅠa 2(2/46),FCDⅠb 4(4/46),FCDⅡa 11(11/46),FCDⅡb13(13/46),FCDⅢa 1(1/46),未分型的FCD8例(8/46)。在3D T_(1)WI图像上由两位影像科医生结合手术病理记录标注FCD/MCD区域作为正样本,并在同一病例大脑半球对侧对称性区域标注对照区域作为负样本。将数据按7:3的比例随机分为训练集和测试集。通过特征提取、特征降维、特征选择、分类器训练等过程,训练影像组学模型。以受试者工作特征曲线(ROC)评价模型效能。结果:46个FCD/MCD病灶的三维径线为3.2 cm(95%CI:2.9-3.4)、4.1 cm(95%CI:3.8-4.4)和5.9 cm(95%CI:5.3-6.5),46个对照病灶的三维径线为4.2 cm(95%CI:3.5-4.7)、4.9 cm(95%CI:4.2-5.5)和6.6 cm(95%CI:5.8-7.3),两两比较差异均无统计学意义(P>0.05)。46个FCD/MCD病灶的体积为17.9 cm^(3)(95%CI:14.4-21.3),46个对照病灶的体积为18.8 cm^(3)(95%CI:14.5-23.2),差异无统计学意义(P>0.05)。训练集AUC为0.997,测试集AUC为0.851。结论:基于3D T_(1)WI图像的影像组学模型对儿童FCD/MCD预测有一定的准确性,有必要进一步研究。Objective:To explore the predictive efficacy of radiomics model of focal cortical dysplasia(FCD)and malformations of cortical development(MCD)based on 3D T_(1) weighted images(T_(1)WI).Methods:This retrospective study collected the patients with FCD from March,2015 to August,2020.Forty-six patients were collected(male 28,female 18,average age 7.2).All patients underwent surgery after MRI examination.The post operation pathology results were collected with 39 cases of FCD(Ⅰa 2,Ⅰb 4,Ⅱa 11,Ⅱb13,Ⅲa 1,undefined 8)and 8 cases of MCD.The labels of FCD/MCD foci were manually annotated by two radiologists as the positive samples.The labels of healthy appearance on the contralateral areas were annotated as the negative samples.The data were randomly divided into training set and test set in the ratio of 7:3.The radiomics model was trained through the process of feature extraction,feature dimension reduction,feature selection,and classifier training.After obtaining the prediction results of the test set,the model efficacy was evaluated by receiver operating characteristic curve(ROC).Results:The three-dimensional diameters of the FCD/MCD foci were 3.2cm(95%CI:2.9-3.4),4.1cm(95%CI:3.8-4.4),and 5.9cm(95%CI:5.3-6.5).The three-dimensional diameters of the FCD/MCD foci were 4.2cm(95%CI:3.5-4.7),4.9cm(95%CI:4.2-5.5),and 6.6cm(95%CI:5.8-7.3).The differences were not significant between the diameter of the FCD/MCD foci and the control areas(all P>0.05).The average volume of the FCD/MCD foci was 17.9cm^(3)(95%CI:14.4-21.3).The average volume of the control lesions was 18.8cm^(3)(95%CI:14.5-23.2).The difference was not significant different between the volume of the FCD/MCD foci and the control areas(P>0.05).The AUCs of the training set and test set were 0.997 and 0.851.Conclusion:This small-scale study proved that the radiomics model based on the 3D T_(1)WI images has certain accuracy for the prediction of FCD/MCD foci,and further study should be performed.
