针刺防控青少年单纯性近视疗效的Meta分析

摘　　要：目的系统评价针刺防控青少年单纯性近视的疗效。方法计算机检索中国期刊全文数据库(CNKI)、维普中文科技期刊全文数据库(VIP)、万方学术期刊全文数据库(Wanfang)、中国生物医学文献数据库(CBM)、PubMed、荷兰医学文摘数据库(Embase)、Cochrane图书馆(Cochrane Library)等数据库中针刺防控青少年单纯性近视的随机对照试验,检索时限截至2021年1月,2位研究者独立对纳入文献进行资料提取和质量评价,使用RevMan5.4.1软件进行Meta分析。结果共纳入16项研究,Meta分析结果显示,针刺防控近视对比其他疗法疗效具有显著性优势[OR=3.91,95%CI:3.05~5.01,P<0.01](滴眼液:OR=3.24,95%CI:1.95~5.38,耳穴:OR=2.57,95%CI:1.10~6.02,推拿:OR=1.97,95%CI:0.95~4.11),且漏斗图结果显示基本对称。Meta分析显示针刺疗法对比其他疗法改善远视力[MD=0.14,95%CI:0.12~0.16,P<0.01]、屈光度[MD=0.01,95%CI:-0.04~0.06,P=0.69]。结论本次系统评价证实针刺防控青少年单纯性近视有效,且能够提高近视患者远视力,尚不能确定能够降低屈光度。但由于本次纳入文献质量较低,仍需要更多设计合理且方法得当的高质量研究加以验证。Objective:To systematically evaluate the effect acupuncture for prevention and control of simple myopia in adolescents.Methods:Computer retrieval was made to search randomized controlled trials of acupuncture in the prevention and control of simple myopia in adolescents from databases such as journals of full-text Database(CNKI),VIP Chinese Science and Technology Periodicals Full-text Database(VIP),Wanfang Academic Journals Full-text Database(Wanfang),China Biomedical Literature Database(CBM),PubMed,Netherlands Medical Abstracts Database(Embase),Cochrane Library,etc.The retrieval time was as of January 2021.Two researchers independently extracted data and evaluated the quality of the included literature,using RevMan5.4.1 Software for Meta analysis.Results:A total of 16 studies were included.The Meta-analysis showed that acupuncture had a significant advantage in the prevention and control of myopia compared with other treatments[OR=3.91,95%CI:3.05-5.01,P<0.01](eye drops:OR=3.24,95%CI:1.95-5.38,auricular point:OR=2.57,95%CI:1.10-6.02,massage:OR=1.97,95%CI:0.95-4.11),and the funnel plot results showed basic symmetry.The Meta-analysis showed that the acupuncture therapy improved distance vision[MD=0.14,95%CI:0.12-0.16,P<0.01]and refraction[MD=0.01,95%CI:-0.04-0.06,P=0.69]compared with other treatments.Conclusion:This systematic review confirms that acupuncture is effective in the prevention and control of simple myopia in adolescents,and can improve the distance vision of myopic patients,but it is uncertain whether it can reduce the refraction.However,due to the low quality of the included literature,more high-quality studies with reasonable design and appropriate methods are still needed for verification.
