自拟清胃疏肝方联合西医常规治疗胃溃疡伴Hp感染的疗效观察及对血清EGF、PGE2水平的影响

摘　　要：目的:探讨自拟清胃疏肝方联合西医常规治疗胃溃疡伴Hp感染的临床疗效及对血清EGF(表皮生长因子)、PGE2(前列腺素E2)水平的影响。方法:选取2019年12月—2021年2月于本院诊治的82例胃溃疡伴Hp感染患者作为研究对象,按照随机数字表法分为对照组(41例)、观察组(41例)。对照组给予奥美拉唑肠溶胶囊、克拉霉素片、阿莫西林胶囊、果胶铋四联用药,观察组予四联用药联合自拟清胃疏肝方治疗。疗程结束后比较两组患者的临床疗效和治疗前后中医症状评分、血清EGF与PGE2变化及Hp清除率。结果:观察组疗效优于对照组(P<0.05);治疗后两组患者胃脘灼热、胃脘疼痛、口干口苦、胸胁胀满、嘈杂泛酸、烦躁易怒等中医症状评分均降低(P<0.05),观察组降低更明显(P<0.05);治疗后两组患者血清EGF、PGE2水平均升高(P<0.05),观察组升高更明显(P<0.05)。观察组Hp清除率明显高于对照组(P<0.05)。结论:自拟清胃疏肝方联合西医常规治疗胃溃疡伴Hp感染患者,可有效减轻患者临床症状,有利于清除Hp;增加EGF与PGE2的分泌是其部分作用途径。Objective:To observe the curative effect of gastric ulcer and Helicobacter pylori infection treated with Qingwei Shugan Decoction combined with Western medicine conventional therapy and explore the effects on serum EGF and PGE2 levels.Methods:82 Patients with gastric ulcer and Helicobacter pylori infection were selected and divided into control group(41 cases)and observation group(41 cases),control group was treated with Omeprazole,Clarithromycin,Amoxicillin and Bismuth pectin,observation group was given Qingwei Shugan Decoction on the basis of control therapy.After the treatment course,the curative effect was evaluated,the symptoms scores,serum EGF and PGE2 levels and Hp clearance rate were compared between the two groups.Results:The curative effect of observation group with Qingwei Shugan Decoction combined with Western medicine was better than that of control group only with Western medicine(P<0.05).The symptoms scores of the two groups were lowered after treatment and those in observation group lowered significantly(P<0.05).Serum EGF and PGE2 levels in the two groups were raised and those in observation group raised significantly(P<0.05).Hp clearance rate of the observation group was higher than that of control(P<0.05).Conclusion:Qingwei Shugan Decoction can effectively raise the curative effect of gastric ulcer and Helicobacter pylori infection treated with Western medicine conventional therapy,effectively alleviate clinic symptoms,that is contribute to clear Hp,promoting the secretion of EGF and PGE2.
