摘 要:目的了解功能性构音障碍(functional articulation disorders,FAD)患儿口部运动功能发育情况,为个体化干预提供参考。方法以113例功能性构音障碍患儿为研究对象,对其进行构音障碍评估,用中国韦氏儿童智力量表进行智力测试,采用口部运动量表进行口部运动功能评估,分析结果。结果113例FAD患儿发音错误率以舌尖后音最高,占62.0%(70/113),其次是舌根音,占56.6%(64/113);错误形式以置换、歪曲为主;口部运动功能评估结果显示,下颌运动异常占15.9%(18/113),唇运动占2.7%(3/113),舌运动占34.5%(39/113),总运动占14.2%(16/113),以舌运动异常最明显;口部各项运动与言语能力呈正相关(r=0.197~0.258,P<0.05),下颌运动、舌运动、总运动与总智力呈正相关(r=0.194~0.251,P<0.05),不同发音部位在各运动项目评分等级之间无明显差异(P>0.05);FAD女性患儿口部总运动优于男性患儿(P<0.05)。结论FAD患儿存在明显口部运动障碍,以舌运动异常为主;FAD患儿应进行精细化口部运动评估,以利指导制定临床干预方案。Objective To investigate the oral motor assessment in children with functional articulation disorders(FAD),in order to provide a theoretical basis for developing individualized interventions.Methods A total of 113 FAD children were recruited,and the assessment results of language,intelligence and oral motor were retrospectively analyzed.Results The mispronunciation of"blade palatou"sounds was the most common(62.0%,70/113)in children with FAD,followed by velar sounds(56.6%,64/113).The main articulation errors were replacement and distortion.The oral motor assessment revealed that the rate of abnormal movement in the mandible,lips,tongue and total motor was 15.9%(18/113),2.7%(3/113),34.5%(39/113)and 14.2%(16/113),respectively.The most obvious motor abnormality was tongue movement.There was positive correlation between mouth motor and verbal ability in children with FAD(r ranged from 0.197 to 0.258).The scores of mandible,tongue and total motor in FAD children was significantly related with overall intelligence scores(r ranged from 0.194 to 0.251).There was no obvious difference between the score of different place of articulation and each motor item.The total oral motor ability in girls was better than that in boys in children with FAD.Conclusion Children with FAD have significant abnormal oral motor development,with difficulty in tongue movement as the main manifestation.Therefore,it is very important to carry out a detailed oral movement assessment in children with FAD for the development of individualized interventions.
