摘 要:病机十九条是中医病机理论的总纲。文章以病机十九条中有关火热病机的原文为据,以隐喻分析为方法,探究《黄帝内经》中火热病机可能的认知根源。发现古人往往借助自然界之火热来认识人体的“火热”,并通过火、热间的差异来认识以“火”“热”为病机的病证。而太阳与火热间的关系映射至人体则表现为心与人体之“火”“热”间的认知关联。临床上,以“火”为主要病机的病证多与神志相关,且与以“热”为主要病机的病证相比,症状体征的范围较小,临床表现更为剧烈。The nineteen provisions for pathogenesis is the general outline of the theory of traditional Chinese medicine pathogenesis. This paper uses the original text of the 19 provisions for pathogenesis regarding fire and heat disease pathogenesis as the basis, and uses metaphorical analysis as a method to explore the possible cognitive roots of fire and heat pathogenesis in the Yellow Emperor’s Classic of Internal Medicine. It was found that the ancients often used the fire and heat of nature to recognize the ‘fire and heat’ of the human body, and the differences between fire and heat to recognize the disease with ‘fire’ and ‘heat’ as the pathogenesis. The relationship between the sun and fire is mapped to the human body as a cognitive association between the heart and the ‘fire’ and ‘heat’ of the human body. Clinically, the symptoms of ‘fire’ as the main pathogenic mechanism are mostly related to the mind, and compared with those of ‘heat’ as the main pathogenesis, the range of symptoms and signs is smaller and the clinical manifestations are more intense.
