摘  要:目的探讨子宫内膜癌中DNA损伤检查点蛋白调节子1(MDC1)和DNA错配修复(MMR)蛋白表达及其临床意义。方法回顾性选取2020年2月至2021年2月青海大学附属医院妇科收治的子宫内膜癌患者100例,采用免疫组织化学染色法检测MDC1和MMR蛋白表达情况,分析MDC1和MMR蛋白表达与临床病理特征的相关性,并分析MDC1与MMR蛋白表达的相关性。结果100例患者中,MDC1阳性90例,阴性10例,MDC1阳性率为90.00%,MDC1阳性患者的年龄高于阴性患者,子宫内膜样腺癌比率低于阴性患者,组织学分级1~2级比率低于阴性患者,差异均有统计学意义(P<0.05)。MMR表达完整(MMR-p)80例,MMR表达缺失(MMR-d)20例,分别占总数的80.00%、20.00%,MMR-p患者的年龄高于MMR-d患者,子宫内膜样腺癌比率、组织学分级1~2级比率均低于MMR-d患者,差异均有统计学意义(P<0.05)。MDC1阳性/MMR-p 78例,MDC1阳性/MMR-d 10例,MDC1阴性/MMR-p 2例,MDC1阴性/MMR-d 10例,分别占总数的78.00%、10.00%、2.00%、10.00%,年龄逐渐降低,组织学类型、组织学分级之间的差异均有统计学意义(P<0.05)。MDC1阳性90例中,强阳性表达65例,弱阳性表达25例,分别占72.22%、27.78%,强阳性表达患者的子宫内膜样腺癌比率低于弱阳性表达患者,非子宫内膜样腺癌比率高于弱阳性表达患者,差异有统计学意义(P<0.05)。MMR-d 20例中,PMS2阴性5例,MSH6阴性3例,MLH1和PMS2阴性9例,MSH2和PMS2阴性1例,MSH2和MSH6阴性2例,分别占25.00%、15.00%、45.00%、5.00%、10.00%。MDC1阴性表达和MMR-d呈显著正相关(r=0.562,P<0.05)。结论MDC1和MMR蛋白主要失表达于低级别子宫内膜癌中,这就为临床鉴别诊断与针对性治疗子宫内膜癌提供了有效依据。Objective To investigate the expression and clinical significance of MDC1 and MMR proteins in endometrial carcinoma.Methods A total of 100 patients with endometrial cancer admitted to the Department of Gynecology,Affiliated Hospital of Qinghai University from February 2020 to February 2021 were retrospectively selected.Immunohistochemical staining was used to detect the protein expression of MDC1 and MMR.The correlation between the protein expression of MDC1 and MMR and clinicopathological features was analyzed.Results Among 100 patients,90 were MDC1 positive and 10 were MDC1 negative,the positive rate of MDC1 was 90.00%,the age of MDC1 positive patients was higher than that of negative patients,the proportion of endometrioid adenocarcinoma and histological grade 1-2 was lower than that of negative patients,which was lower than that of negative patients,the difference was statistically significant(P<0.05).80 cases with complete MMR expression(MMR-p)and 20 cases with MMR expression loss(MMR-d),accounting for 80.00%and 20.00%of the total,respectively,the age of MMR-p patients was higher than that of MMR-d,and the proportion of endometrioid adenocarcinoma and histological grade 1-2 were lower than those of MMR-d,and the differences were statistically significant(P<0.05).MDC1 positive/MMR-P78 cases,MDC1 positive/MMR-D10 cases,MDC1 negative/MMR-P2 cases,MDC1 negative/MMR-D10 cases,accounting for 78.00%,10.00%,2.00%,10.00%,respectively,the age gradually decreased,and the differences between histological types and histological grades were statistically significant(P<0.05).Among 90 MDC1 positive patients,65 cases were strongly positive and 25 cases were weakly positive,accounting for 72.22%and 27.78%,respectively.The proportion of endometrioid adenocarcinoma in patients with strongly positive expression was lower than that in patients with weakly positive expression,and the proportion of non-endometrioid adenocarcinoma was higher than that in patients with weakly positive expression(P<0.05).Among the 20 mMR-D patients,5
