摘 要:目的 探讨循证医学(evidence-based medicine,EBM)结合以问题为基础的教学法(problem-based learning,PBL)在心内科临床教学中的应用效果。方法2016年9月—2018年9月从川北医学院2014级临床医学本科入学新生中遴选出30名学生组建成“卓越医生班”,作为试验组(30名),采用EBM结合PBL教学法;随机选择同年级临床医学本科班学生作为对照组(30名),采用以授课为基础的教学法(lecture-based learning,LBL)。对两组的教学效果进行比较。结果 试验组理论考试成绩为(84.3±5.0)分,高于对照组的(78.8±4.8)分;试验组技能考核成绩为(82.6±5.4)分,高于对照组的(76.5±4.9)分,差异均有统计学意义(P<0.05)。问卷调查结果显示试验组学生在激发学习兴趣与自主性、查阅文献资料能力、掌握基本理论基础知识能力、提高临床思维能力、提高团队协作能力及提高分析与解决问题能力均高于对照组,差异有统计学意义(P<0.05)。结论 与LBL相比,EBM结合PBL教学心血管疾病研究法更能激发学生自主学习的兴趣,更能提高学生的理论和技能成绩及解决临床实际问题的能力,有利于培养医学生的团队协作能力及综合能力,可广泛应用。Objective To explore the application effect of EBM Combined with PBL in clinical teaching of cardiology.Methods From September 2016 to September 2018, 30 students were selected from the class of 2014 undergraduate students of clinical medicine of North Sichuan Medical College to form the "excellent doctor class" as the experimental group(30 students),using the EBM and PBL teaching method;The control group(30 students) was randomly selected from the clinical medical undergraduate class of the same grade, and lecture-based learning(LBL) was adopted. Compare the teaching effect of the two groups. Results The score of theoretical examination in the experimental group was(84.3±5.0) points, higher than that in the control group(78.8±4.8). The skill examination score of the test group was(82.6±5.4) points, which was higher than that of the control group(76.5±4.9) points, and the differences were statistically significant(P<0.05).The results of the questionnaire survey showed that the students in the test group were significantly higher than those in the control group in stimulating learning interest and autonomy,consulting literature, mastering basic theoretical knowledge,improving clinical thinking ability, improving teamwork ability, and improving the ability to analyze and solve problems(P<0.05). Conclusion Compared with LBL, EBM combined with PBL teaching cardiovascular disease research method can stimulate students’ interest in independent learning, improve students’ theoretical and technical performance and ability to solve clinical practical problems, which is conducive to cultivate medical students’ team cooperation ability and comprehensive ability, and can be widely used.
