摘 要:目的研究程序性死亡受体1(PD-1)基因单核苷酸多态性与肝细胞癌临床表型是否存在相关性。方法纳入2021年1—12月南通大学附属医院介入放射科收治的新发肝细胞癌患者165例,采集基因组DNA,利用panel高通量数据分析技术对PD-1基因进行分型,分析各基因型与肝细胞癌临床表型之间的相关性。结果PD-1基因rs36084323位点CT基因型患者较野生基因型CC患者分期偏晚的风险降低(OR=0.306,95%CI:0.118~0.679,P<0.05),发生门静脉侵犯的风险降低(OR=0.239,95%CI:0.098~0.631,P<0.05),出现肝外转移的风险降低(OR=0.394,95%CI:0.165~0.833,P<0.05)。PD-1基因rs10204525位点、rs2227982位点不同基因型与肝癌巴塞罗那分期(BCLC)、门静脉侵犯、肝外转移等临床表型之间无相关性(P>0.05)。结论PD-1基因rs36084323位点单核苷酸多态性与肝细胞癌临床表型相关。Objective To investigate the correlation between the single nucleotide polymorphisms of programmed death receptor 1(PD-1)gene and different clinical phenotypes of hepatocellular carcinoma.Methods From January to December 2021,165 patients newly diagnosed with hepatocellular carcinoma who were admitted to the Department of Interventional Radiology of the Affiliated Hospital of Nantong University were selected as enrolled subjects to extract genomic DNA.The panel high-throughput data analysis was adopted to classify PD-1 genotyping and the correlation between each genotype and the clinical phenotype of hepatocellular carcinoma was analyzed.Results At locus rs36084323,patients with genotype CT had a lower risk of later stage than patients with wild-type CC(OR=0.306,95%CI:0.118-0.679,P<0.05),the patients with genotype CT had a lower risk of portal vein invasion than patients with wild-type CC(OR=0.239,95%CI:0.098-0.631,P<0.05),the patients with genotype CT had a lower risk of extrahepatic metastasis compared with patients with wild-type CC(OR=0.394,95%CI:0.165-0.833,P<0.05).There were no significant differences in the clinical phenotypes of hepatocellular carcinoma such as BCLC stage,portal vein invasion and extrahepatic metastasis among all genotypes at loci rs10204525 and rs2227982(P>0.05).Conclusion The single nucleotide polymorphisms at rs36084323 of PD-1 gene are correlated with clinical phenotypes of hepatocellular carcinoma.
