摘　　要：目的对疾病诊断相关分组(diagnosis related groups,DRG)精细化管理效果进行评价并针对存在的问题提出改进措施。方法选取2019—2021年医院的住院病案首页数据,运用CN-DRG分组方案进行分组,选取广东省DRG评价指标体系中的DRG组数、病例组合指数(case mix index,CMI)、总权重、时间消耗指数、费用消耗指数和低风险组死亡率6个指标对医院精细化管理进行纵向评价。结果2019—2021年,医院的总权重逐步上升,但CMI值逐步降低,时间消耗指数和费用消耗指数呈下降趋势,低风险组死亡率逐年下降。结论基于DRG的医院精细化管理,可以激励医院提升服务能力、增强服务效率、规范医疗行为和提高医疗安全。Objective To evaluate the effect of the refined management based on disease diagnosis-related groups(DRG),and to propose rational improvement measures to solve possible problems.Methods The home page data of inpatient medical records of the hospital from 2019 to 2021 were selected,and the CN-DRG grouping scheme was used for grouping.The number of DRG groups and Case Mix Index in the DRG evaluation index system of Guangdong Province were selected.CMI,total weight,time consumption index,cost consumption index and mortality rate of low-risk group were used for longitudinal evaluation of hospital fine management.Results From 2019 to 2021,the total weight of the hospital gradually increased,but the CMI value gradually decreased,the time consumption index and cost consumption index show a downward trend,and the mortality rate of the low-risk group decreased year by year.Conclusion The refined management of hospitals based on DRG can motivate hospitals to improve service capabilities,enhance service efficiency,standardize medical practices,and improve medical safety.
