急腹症导向教学对提高住院医师诊断胃肿瘤术后并发症水平的探讨

摘　　要：目的通过回顾上海交通大学医学院附属瑞金医院(简称我院)胃肠外科接受住院医师规范化培训(简称住培)的各学科住院医师的学科分类组成的不同,结合胃肠外科围手术期特点,分析研究将外科急腹症的诊治作为核心课程在外科住培中的优势和推广价值。方法收集2010年8月至2020年8月在我院胃肠外科接受住培的住院医师,分为观察组和对照组,比较两组住院医师之间对于胃肿瘤术后并发症的诊断得分差异。结果我院胃肠外科住院医师共计449人,在出科考核时对于胃肿瘤术后并发症的诊断准确性,接受过系统的以急腹症诊治为核心的临床教学课程的住院医师(观察组)较未接受过相关课程的住院医师(对照组)明显增高(P<0.05)。结论在胃肠外科以急腹症为核心的临床教学对胃肿瘤手术后并发症的诊断能力提升有着最直观和有效的培训效果。Objective To investigate the advantages and promotion value of setting the diagnosis and treatment of acute abdomen as the core course in standardized residency training according to a retrospective study from Ruijin Hospital.Methods Residents trained in gastrointestinal surgery were divided into observation and control groups.T-test was used to determine the differences between two groups of the residents who had received the systematic clinical teaching course aiming at the diagnosis and treatment of acute abdomen.Results Of the 449 residents,the diagnostic ability of postoperative complications of gastric tumors in the observation group receiving the core course in diagnosis and treatment of acute abdomen was significantly higher than that of control group.Conclusion Carrying out clinical teaching courses aiming at acute abdomen has the most intuitive and effective training effect on improving the comprehensive clinical diagnosis and treatment ability for residents and the promotion of clinical competence.
