摘 要:目的探讨横聘杆对青少年患者垂直向生长控制的效果。方法本回顾性研究纳人75例拔除4颗第一前磨牙的青少年高角患者,将患者分为横聘杆组和未使用横聘杆组,通过头影测量分析两组患者治疗前后垂直向的变化。结果两组患者经过治疗后,下颌升支长度、前后面高度、上下颌第一磨牙高度均较治疗前显著增加。其中,横聘杆组下颌升支长度、后面高和前下面高分别增加(2.36±2.30)mm(t=6.23.P<0.001)、(2.71±2.56)mm(t=6.44,P<0.001)和(2.20±2.23)mm(t=5.97,P<0.001),上、下颌第一磨牙高度分别增加(1.66土1.18)mm(t=8.52,P<0.001)和(2.26±1.35)mm(t=10.16,P<0.001),下颌平面角(SN-MP)增加0.62°±1.34°(t=2.83,P=0.01)。未使用横聘杆组下颌升支长度、后面高、前下面高、上下颌第一磨牙高度变化分别为(2.28±2.20)mm(t=7.06,P<0.001)、(2.50±2.20)mm(t=7.01,P<0.001)、(2.19±2.15)mm(t=6.28,P<0.001)、(1.27±1.16)mm(t=6.76.P<0.001)和(2.48±1.12)mm(t=13.57 P<0.001).下颌平面角(SN-MP)增加0.32°±1.53(t=1.28.P=0.21)。两组之间各项垂直向测量指标治疗前后的变化均无显著统计学意义。结论对于减数4颗第一前磨牙的青少年高角患者,横聘杆未能有效控制垂直向高度。无论是否使用横聘杆,两组患者均出现上下颌第一磨牙高度的增加,且部分患者出现下颌平面的顺时针旋转。Objective To observe the effect of the transpalatal arch on the vertical growth control of adolescent patients.Methods sSeventy-five high-angle adolescent patients with extraction of four first premolars were included in this retrospective study.The patients were divided into the transpalatal arch(TPA)group and the non-transpalatal arch(non-TPA)group,and the vertical changes of the two groups during treatment were analyzed by cephalometric measurement.ResultsSignificant increases were found in the length of the mandibular ramus,the anterior and posterior facial heights,as well as the height of the first molars after treatment in both groups.In the TPA group,the mandibular ramus length,the posterior and lower anterior facial height were increased by(2.36±2.30)mm(t=6.23,P<0.001),(2.71±2.56)mm(t=6.44,P<0.001)and(2.20±2.23)mm(t=5.97,P<0.001);the upper and lower first molars height were increased by(1.66±1.18)mm(t=8.52,P<0.001)and(2.26±1.35)mm(t=10.16,P<0.001),respectively,and the mandibular plane(SN-MP)was increased by 0.62°±1.34°(t=2.83,P=O.O1).In the non-TPA group,the length of the mandibular ramus,the posterior and lower anterior facial heights,and the height of the maxilary and mandibular first molars were increased by(2.28±2.20)mm(t=7.06,P<0.001),(2.50±2.20)mm(t=7.01,P<0.001),(2.19±2.15)mm(t=6.28,P<0.001),(1.27±1.16)mm(t=6.76,P<0.001)and(2.48±1.12)mm(t=13.57,P<0.001),respectively;Meanwhile,the mandibular plane(SN-MP)was increased by 0.32°±1.53°(t=1.28,P=0.21).There was no significant difference in the changes of the vertical measurement indices between the two groups during treatment.ConclusionsFor adolescent high-angle patients with four first premolars extraction,the transpalatal arch failed to effectively control the vertical height and the eruption of the first molar was observed in both groups and the clockwise rotation of the mandibular plane mas also observed in some patients.
