TKI药物治疗儿童CML获得主要分子学反应前后生活质量的变化研究

摘　　要：目的研究儿童慢性髓系白血病(CML)患者接受酪氨酸激酶抑制剂药物(TKI)治疗获得主要分子学反应(MMR)前后生活质量的变化,并分析生活质量改善的影响因素。方法收集2014年1月—2019年12月在首都医科大学附属北京儿童医院住院或门诊CML患儿于初诊时及获得MMR后分别填写的儿童生存质量评价量表PedsQL 4.0(生理功能、情感功能、社交功能、学校功能)、PedsQL 3.0癌症模块(疼痛或受伤、恶心、操作过程的焦虑、治疗焦虑、认知问题、对外貌的自我感觉、交流问题),以及患儿家庭文化程度、家庭年收入、付费方式、依从性,分析其与生活质量影响的关系。结果获得MMR后CML患儿在生理功能、情感功能及学校功能(P<0.05)方面均较初诊时有明显改善,对原发病及治疗过程中的不良反应如疼痛及恶心(P<0.05),对操作及治疗的焦虑(P<0.05)均有很大程度的缓解。但是患儿年龄、性别及付费方式对生活质量无显著影响。结论初发CML患儿应用TKI药物可在控制病情的基础上,极大改善其生活质量。Objective To examine the effects of TKI on health-related quality of life in children with CML via major molecular response(MMR),and to analyze the factors causing the improvement of quality of life.Methods Inpatient or outpatient CML children in Beijing Children′s Hospital of Capital Medical University from January 2014 to December 2019 were recruited and asked to fill out PedsQL 4.0(Physiological Function,Emotional Function,Social Function,and school function),PedsQL 3.0 cancer module(pain or injury,nausea,anxiety during operation,treatment anxiety,cognitive problems,self-perception of appearance,and communication problems),as well as other questionnaires(the children′s family education level,family annual income,payment method,and compliance)which might be related to health-related quality of life.Results After obtaining MMR,the physiological function,emotional function,and school function of children with CML significantly improved(P<0.05).Moreover,the adverse events during TKI treatment,such as pain,nausea,and anxiety about operation and treatment,were relieved to a great extent(P<0.05).The age,gender,and payment method,however,had no significant impact on the quality of life.Conclusions TKI drugs effectively improve health-related quality of life in children with primary CML.
