摘  要:目的通过了解全科医学定向生毕业后的工作现状,分析、讨论其在基层发展的主要制约因素,为桂东北地区全科医学定向生培养提供建议。方法选取桂林医学院2015、2016、2017级临床医学全科医学方向订单定向医学生150人为调查对象,采用自制问卷并通过问卷星的方式,对其毕业后的履约情况、编制落实情况、工作满意度、职称晋升情况、担任职务情况、收入情况、继续教育意愿以及服务期满后基层服务意愿等进行调查。结果收回有效问卷130份,其中履约率达100.00%,已落实编制率为99.23%,全科医生分布在广西桂林、柳州、来宾、贺州、梧州5个桂东北地区,均在农村卫生院工作。在工作单位担任中层职务的比率为27.69%,2015级考取主治医师率为77.27%,但工作及薪酬满意率、服务期满希望继续留任比率较低。结论桂东北地区的全科医学定向生履约率高,在基层工作中有优势,能够发挥重要作用,但基层工作薪资偏低,工作满意度不高,继续教育机会少,难以留住更多的全科医学人才。建议加强政策引导,完善服务措施,落实激励机制,保障全科医生在本地区农村基层稳定可持续发展。Objective To analyze and discuss the main constraints of the development at community-level by surveying the status quo of targerted-area GP graduates at work,and to make suggestions for the training of targerted-area GP graduates in northeast Guangxi.Methods A total of 150 targeted-area medical students of grade 2015,2016 and 2017 in Guilin Medical College were selected as the survey objects.The self-made questionnaire was used to investigate their situation of contract fulfillment,implementation of regimentation,job satisfaction,promotion of professional titles,position,income,willingness to continue education and serve at community-level after the expiration of service.Results A total of 130 valid questionnaires were collected,of which the rate of fulfilling contract was 100.00%,and the rate of acquiring regimentation was 99.23%.GPs were distributed in Guilin,Liuzhou,Laibin,Hezhou and Wuzhou of Guangxi,all of which worked in rural hospitals.The proportion of middle-level positions in hospitals was 27.69%,and the rate of physician-in-charge in the 2015 grade was 77.27%.However,there were the low satisfaction rate of work and salary and the low rate of willing to continue after the expiration of service.Conclusion The contract fulfillment rate of targeted-area GP students in northeast Guangxi is high,and they have advantages in community-level work and can play an important role.However,the low salary of coummity-level work,the not high job satisfaction,and the few opportunities for continuing education result in that it is difficult to retain more GPs.It is recommended to strengthen policy guidance,improve service measures,implement incentive mechanisms,so as to ensure the stable and sustainable development of GP in rural community-level.
