摘 要:目的结合住院医师临床技能操作岗前培训前后的两次考核结果,探讨住院医师规范化岗前培训计划的培训实践效果。方法对我院2018~2021年期间招录的261名住培医师进行临床技能岗前培训和两次循环式操作考核。比较培训前后的考核合格率和成绩,总结培训实践经验,分析培训前后存在的问题并探讨优化措施。结果纳入分析的261名住院医师培训后考核总体合格率为96.60%,较培训前总体合格率存在明显统计学差异(率差值0.88,95%CI 0.84~0.92,P<0.05)。每个年级各项技能考核合格率在培训前后均有明显统计学差异(P均<0.05)。培训后考核总体平均中位成绩为85.88(81.59,89.56)分,较培训前总体平均成绩分布存在明显统计学差异(差值31.14分,95%CI 29.72~32.56,H=19.42,P<0.05)。每个年级各项技能操作成绩和平均成绩分布在培训前后均有明显差异(P均<0.05)。基础生命支持理论考核中位成绩为96.00(92.00,100)分。培训前最薄弱技能项为急救技能(30.30%),培训后为专科技能(27.60%)。结论岗前技能培训后的考核合格率和成绩提升幅度显著,但针对薄弱技能项应该进一步优化培训模式,制定个性化培训计划。Objective To explore the training practice effect of standardized pre-post training program for resident physicians in combination with the results of two assessments before and after pre-job training for clinical skill operation.Methods A total of 261 resident physicians recruited in our hospital from 2018 to 2021 were trained in clinical skills before work and assessed twice in cyclic operation.The qualified rate and achievement of assessment before and after training were compared,the practical experience of training was summarized,the existing problems before and after training and discuss the optimization measures were analyzed.Results The overall qualified rate of 261 residents included in the analysis was 96.60%after training,which was significantly different from the overall qualified rate before training(rate difference 0.88,95%CI 0.84-0.92,P<0.05).There were significant differences in the qualified rate of each grade before and after training(P<0.05).The overall mean score of assessment after training was 85.88(81.59,89.56)points,which was significantly different from the overall mean score distribution before training(difference 31.14 points,95%CI 29.72-32.56,H=19.42,P<0.05).There were significant differences in the distribution of skill operation scores and average scores before and after training in each grade(P<0.05).The median score of basic life support theory assessment was 96.00(92.00,100)points.The weakest skill items before training were first aid skills(30.30%)and specialist skills after training(27.60%).Conclusion The qualified rate of assessment and the improvement range of performance after pre-job skill training are significant,but the training mode should be further optimized for weak skill items,and a personalized training plan should be formulated.
