摘 要:目的探讨以案例为导向的教学法(case-based learning,CBL)联合以问题为导向的教学法(problem-based learning,PBL)在提高进修医师头颈部影像诊断能力中的应用价值。方法选取2019年度(2019年3月—2020年3月)及2020年度(2020年3月—2021年3月)共70名在西安交通大学第一附属医院学习头颈部影像诊断的进修医师,其中2019年进修医师36名纳入对照组,接受传统以授课为导向的教学法(lecture-based learning,LBL)。2020年进修医师34名归为试验组,接受CBL联合PBL教学法。教学活动结束后对两组进修医师进行头颈部影像理论基础和读片能力的考核,并通过调查问卷评估教学满意度。结果试验组进修医师的头颈部影像理论知识和诊断技能的成绩均高于对照组,差异有统计学意义(P<0.05)。同时,调查问卷表明,无论关于头颈部影像基础知识还是诊断能力,试验组进修医师对CBL联合PBL教学法的满意度均高于对照组对LBL教学的满意度,两组差异有统计学意义(P<0.05)。结论CBL联合PBL教学法有助于巩固进修医师在头颈部的影像理论基础,提高诊断能力,获得良好的教学满意度,可以在一定范围内应用。Objective To explore the application value of case-based learning(CBL)combined with problem-based learning(PBL)teaching method in improving the imaging diagnostic capability of the head and neck diseases for refresher doctors.Methods A total of 70 refresher physicians who studied head and neck imaging diagnosis in the First Affiliated Hospital of Xi’an Jiaotong University in 2019(March 2019 to March 2020)and 2020(March 2020 to March 2021)were selected,of which 36 refresher physicians in 2019 were included in the control group and accepted the traditional lecture-based learning(LBL).In 2020,34 refresher physicians were assigned to the experimental group and accepted the CBL combined with PBL teaching method.After the teaching activities,the theoretical basis of head and neck imaging and the ability to read films were assessed for the two groups of advanced physicians,and the teaching satisfaction was assessed through questionnaires.Results The scores of the head and neck imaging theory knowledge and diagnostic skills of the refresher physicians in the test group were higher than those in the control group,and the difference was statistically significant(P<0.05).At the same time,the questionnaire showed that,regardless of the basic knowledge of head and neck imaging or diagnostic ability,the satisfaction of the refresher physicians in the test group with CBL combined with PBL teaching method was higher than that of the control group with LBL teaching method,and the difference between the two groups was statistically significant(P<0.05).Conclusion CBL combined with PBL teaching method are useful to consolidate the theoretical basis and improve the imaging diagnostic capability in head and neck lesions,as well as achieve satisfaction from most of the refresher doctors,which can be popularized in a certain degree.
