摘 要:目的探索基于Debriefing的模拟教学在医学硕士专业学位研究生(简称专硕)妇科腹腔镜培训中的应用。方法选取2021年―2022年南通大学附属医院妇产科临床技能培训中心培训的80名专硕,完成妇科腹腔镜模拟训练后,进行腹腔镜输卵管妊娠的情景模拟手术教学,并随机分为Debriefing教学组(观察组)和未行Debriefing教学组(对照组),各40人。比较两组专硕利用DASH量表评估基于Debriefing的模拟教学的实施效果、腹腔镜进腹速度、电凝电切设备的应用效果;收集专硕对教学方法满意度、教学体验调查表。结果两组DASH量表结果数据显示,观察组在DASH量表的5个方面的得分均高于对照组,均具有统计学意义(均P<0.05);观察组腹腔镜进腹速度所需时间少于对照组[(9.35±1.55)min比(11.60±2.33)min,P<0.05];观察组完成电凝电切训练考核操作的表现优于对照组(P<0.05);调查问卷显示观察组对教学方式的满意程度高于对照组(P<0.05)。结论基于Debriefing的模拟教学可有效提高专硕妇科腹腔镜模拟技能培训的效率,减少操作失误,从而降低不良事件的发生,缩短学习曲线,更易被专硕所接纳。Objective To explore the application of Debriefing-based simulation teaching in gynecological laparoscopic training.Methods A total of 80 professional postgraduates who were trained in the Clinical Skills Training Center of Obstetrics and Gynecology,Affiliated Hospital of Nantong University from 2021 to 2022 were chosen to take part in the experiment.They were arranged to continue doing a scene simulation teaching of laparoscopic for an ectopic pregnancy case after completing the gynecological laparoscopy simulation model training.Then they were randomly divided into Debriefing Teaching Group(experimental group,n=40)and no Debriefing Teaching Group(control group,n=40).In this way,two groups of students,using DASH scale,were compared to evaluate the implementation effect of Debriefing-based simulation teaching.The two groups of students were compared in laparoscopic abdominal speed,application effect of electrocoagulation and electrocutting equipment,and a questionnaire was used to compare students’satisfaction with teaching and studying experience.Results DASH in both groups showed that the experimental group scored higher than the control group in five aspects.Comparing the two groups in laparoscopy,the time required in the experimental group[(9.35±1.55)m vs(11.60±2.33)m]was less than that in the control group(P<0.05).The results of electrocoagulation and electrocutting training showed that the experimental group performed better than the control group(P<0.05).The questionnaire showed that the students in the experimental group were more satisfied with the teaching method than the control group.Conclusion Debriefing-based simulation teaching can effectively improve the efficiency of gynecological laparoscopic simulation skills training,reduce the error rate of operation,and thus lead to the decrease of adverse events,which is more acceptable to students and shortens the learning curve.
