成人急性淋巴细胞白血病患者中FLT3基因突变特点及其意义

摘　　要：目的 分析成人急性淋巴细胞白血病(ALL)患者FLT3基因突变特点与其意义。方法 选取123例成人ALL患者。运用聚合酶链式反应(PCR)扩增产物,以限制性酶切产物分析法了解成人ALL患者FLT3基因突变现象,探究FLT3基因与患者临床病理特征的关系;随访3年,探究该基因突变与此类患者预后的关系。结果 123例成人ALL患者内,FLT3基因突变的有14例,突变率为11.38%(14/123),突变类型内,FLT3-ITD突变3例(21.43%),FLT3-TKD突变11例(78.57%);FLT3基因突变与FLT3基因无突变患者的年龄、性别、血小板计数(PLT)、白细胞计数(WBC)、血红蛋白(Hb)、乳酸脱氢酶(LDH)、骨髓原始细胞比率、淋巴细胞绝对值(LYM)、外周血原始细胞比率相比,无统计学差异(P>0.05);FLT3基因突变71.43%(10/14)与FLT3基因无突变83.74%(103/123)患者的3年生存率相比,无统计学差异(P>0.05)。结论 FLT3基因突变在成人ALL患者中较为少见,且与患者的生存率无明显关系。Objective To analyze the characteristics and significance of FLT3 gene mutations in adult acute lymphoblastic leukemia(ALL) patients.Methods 123 adult ALL patients were selected.Polymerase chain reaction(PCR) amplification products were used to understand the FLT3 gene mutation in adult ALL patients by restriction enzyme digestion product analysis, and to explore the relationship between FLT3 gene and clinical pathological characteristics of patients;Follow up for 3 years to explore the relationship between the gene mutation and the prognosis of such patients.Results Among 123 adult ALL patients, there were 14 cases of FLT3 gene mutation, the mutation rate was 11.38%(14/123),3 cases of FLT3-ITD mutation(21.43%) and 11 cases of FLT3-TKD mutation(78.57%);There was no significant difference in age, sex, platelet count(PLT),white blood cell count(WBC),hemoglobin(Hb),lactate dehydrogenase(LDH),bone marrow primordial cell ratio, lymphocyte absolute value(LYM) and peripheral blood primordial cell ratio between FLT3 gene mutation and FLT3 gene non-mutation patients(P>0.05);There was no significant difference in the 3-year survival rate between 71.43%(10/14) patients with FLT3 gene mutation and 83.74%(103/123) patients without FLT3 gene mutation(P>0.05).Conclusion FLT3 gene mutation is rare in adult ALL patients, and has no significant relationship with the survival rate of patients.
