益气清热解毒汤联合蒙脱石散治疗直肠癌术后腹泻临床研究

摘　　要：目的:观察益气清热解毒汤联合蒙脱石散治疗直肠癌术后腹泻的临床疗效。方法:将直肠癌术后腹泻患者96例按随机数字表法分为观察组与对照组各48例。对照组以蒙脱石散口服治疗,观察组在对照组的基础上辅以自拟益气清热解毒汤治疗。治疗14 d后,观察比较2组临床疗效及不良反应发生情况,比较治疗前后血清应激反应指标[C-反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素(IL-8)、超氧化物歧化酶(SOD)、丙二醛(MDA)],内脏蛋白指标[前白蛋白(PA)、转铁蛋白(TRF)]水平。结果:观察组临床总有效率为95.83%,高于对照组75.00%(P<0.05)。治疗后,2组血清CRP、IL-6、IL-8及血浆MDA水平均较治疗前降低,2组比较,差异有统计学意义(P<0.05),且观察组各项指标均低于对照组(P<0.05);2组血浆SOD水平均较治疗前升高(P<0.05),且观察组SOD水平高于对照组(P<0.05)。治疗后,2组血清PA、TRF水平均较治疗前升高(P<0.05),且观察组2项指标均高于对照组(P<0.05)。治疗期间,观察组不良反应发生率为4.17%;对照组为6.25%。2组比较,差异无统计学意义(P>0.05)。结论:益气清热解毒汤联合蒙脱石散治疗直肠癌术后腹泻能提高临床疗效,降低机体应激与炎症反应,改善内脏蛋白水平,且安全性良好。Objective:To observe the clinical effect of Yiqi Qingre Jiedu Decoction combined with Montmorillonite Powder for diarrhea after operation for rectal cancer.Methods:A total of 96 cases of patients with diarrhea after operation for rectal cancer were divided into the observation group and the control group according to the random number table method,with 48 cases in each group.The control group was given the oral administration of Montmorillonite Powder,and the observation group was additionally treated with self-made Yiqi Qingre Jiudu Decoction based on the treatment of the control group.After 14 days of treatment,the clinical effects and the incidences of adverse reactions in the two groups were observed and compared.Before and after treatment,the levels of serum stress response indexes including C-reactive protein(CRP),interleukin-6(IL-6),interleukin-8(IL-8),superoxide dismutase(SOD)and malondialdehyde(MDA)as well as visceral protein indexes including prealbumin(PA)and transferrin(TRF)in the two groups were compared.Results:The total effective rate of clinical effect was 95.83% in the observation group,higher than that of 75.00% in the control group,the difference being significant(P<0.05).After treatment,the levels of CRP,IL-6 and IL-8 in serum and plasma MDA in the two groups were decreased when compared with those before treatment(P<0.05),and all the above indexes in the observation group were lower than those in the control group(P<0.05).The levels of plasma SOD in the two groups were increased when compared with those before treatment(P<0.05),and the level in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of PA and TRF in the two groups were increased when compared with those before treatment(P<0.05),and the above two indexes in the observation group were higher than those in the control group(P<0.05).During treatment,the incidence of adverse reactions was 4.17% in the observation group and 6.25% in the control group,there being no significant difference b
