摘 要:目的:探讨以问题为基础学习(PBL)教学模式在神经重症监护室(NICU)重型aSAH患者临床教学中的教学经验。方法:共纳入27名住院医师规范化培训学员作为教学对象,随机入组PBL教学组或传统教学组(LBL)。选择Hunt-Hess分级Ⅲ-Ⅳ级的动脉瘤性蛛网膜下腔出血(aSAH)的患者作为教学病例。两组学员分别采取PBL教学法和LBL教学法,并通过理论考试及问卷调查进行两组教学效果评价。结果:PBL组在两个一级指标(理论考试与问卷调查)总分及16项二级指标单项得分均显著高于LBL组,差异具有统计学意义。结论:在NICU通过PBL对学员进行aSAH患者的临床教学质量得到了验证,提高了学员临床思维能力、实践能力、主动学习能力,可以在NICU住院医师规范化培训的教学中推广。Objective:To explore the teaching experience of problem-based learning(PBL)in the clinical teaching of severe aSAH in the neurointensive care unit(NICU).Methods:A total of 27 residents were randomly divided into PBL teaching group or traditional teaching group(LBL).Aneurysmal subarachnoid hemorrhage(aSAH)patients with Hunt-Hess gradeⅢ-Ⅳwere selected as teaching cases.The two groups of students took PBL teaching method and LBL teaching method respectively.The teaching effect of the two groups was evaluated by theoretical examination and questionnaire survey.Results:The total scores of two first-level indicators(theoretical examination and questionnaire survey)and the individual scores of 16 second-level indicators in PBL group were significantly higher than those in LBL group,and the differences were statistically significant.Conclusion:The quality of PBL teaching on aSAH has been verified in NICU,which can improve the clinical thinking ability,practical ability and active learning ability of students,and can be promoted in the teaching of resident standardized training students in NICU.
