摘　　要：微小RNA(miRNA)通过加速mRNA降解或阻断mRNA翻译来调控相应的靶基因表达。免疫系统作为一种外在的肿瘤抑制因子,通过破坏正在发展的肿瘤或抑制肿瘤扩张来影响肿瘤发生进展。近年来,肿瘤免疫治疗作为肿瘤临床研究的热点正在迅猛发展,免疫检查点抑制剂如PD-1以及其配体PD-L1抑制剂,已在妇科恶性肿瘤中取得了突破性的进展。目前,已有研究发现肿瘤中PD-L1的表达可能受多种miRNA的直接调控,miRNA可能通过肿瘤微环境中免疫状态改变、逆转肿瘤化疗耐药、调节肿瘤的免疫抵抗、诱导杀伤细胞靶向杀伤作用等多个方面调控PD-L1表达,从而影响肿瘤发展的病理过程。miRNA调控PD-L1表达在妇科肿瘤中的研究多集中在宫颈癌、卵巢癌及子宫内膜癌等常见妇科肿瘤。本研究拟综述当前微小RNA调控PD-L1表达在妇科肿瘤中的发展趋势和研究重点,以期为妇科肿瘤患者的临床治疗提供可靠的理论依据。MicroRNAs(miRNAs)regulate target gene expression by accelerating mRNA degradation or blocking mRNA translation.Immune system,as an external tumor suppressor,affects the occurrence and progression of tumors by destroying tumors that are developing or inhibiting tumor expansion.In recent years,tumor immunotherapy is developing rapidly as a hotspot of tumor clinical research.Immune checkpoint inhibitors include programmed cell death-1(PD-1)and its PD-L1 inhibitors have made a breakthrough in gynecological malignancies.At present,it has been found that the expression of PD-L1 in tumors may be directly regulated by multiple miRNAs and miRNA may regulate PD-L1 expression by changing the immune status in tumor microenvironment,reversing tumor chemotherapy resistance,regulating tumor immune resistance,and inducing targeted killing effect of killer cells,thus affecting the pathological process of tumor development.The study on miRNA regulating PD-L1 expression in gynecological tumors mainly focuses on cervical cancer,ovarian cancer,endometrial cancer and other common gynecological tumors.This study reviewed the current development trend and research focus of microRNA-regulated PD-L1 expression in gynecological tumors,in order to provide reliable theoretical basis for clinical treatment of gynecological tumors.
