刍议三脘穴  

A Humble Opinion on Sanwan Points

摘　　要：三脘穴为上脘、中脘、下脘三穴的统称,为任脉腧穴。笔者通过文献梳理与随师临证观察,结合针灸古医籍中三穴定位的描述及近现代研究成果,认为三脘穴定位存在差异的原因为胸骨剑突长短不一。胃有三脘之分,三脘穴对应胃之三脘,功效侧重各有不同,其中,上脘,主受纳,抑而降之,治胃且理胸膈,兼可调神;中脘,主腐熟,和而消之,治胃兼理中气,兼可袪痰;下脘,主通降,散而袪之,治胃兼通肠腑,兼可止痛。从三脘穴对应三焦这一角度进一步分析三穴气血盛衰的不同。三穴深层为重要脏器,临床针刺前应谨慎查体,因人制宜,采取合适的针刺方法。六腑以通为用,以降为和,故临床针刺三穴以泻法居多。基于以上角度,解读及分析三脘穴以期深入挖掘三穴的临床价值。Sanwan points are collectively referred to the three points of Shangwan(CV13),Zhongwan(CV12)and Xiawan(CV10),which belongs to the Ren meridian.Through literature review and clinical observation,combined with the description of location of three points in ancient medical books of acupuncture and moxibustion and modern research results,the author believes that the reason for the difference in location of Sanwan points is the different length of sternum xiphoid process.The stomach is divided into three cavities,and the three cavities correspond to the three cavities of the stomach,and their functions are different.Among them,CV13,governingintake,suppresses and descends,treats the stomach and regulates the chest and diaphragm,and also regulates the mind;CV12,governing decomposition,harmonizes and eliminates,treats the stomach and regulates the middle Qi,and also expels phlegm;CV10,governing the circulation and descending,disperses and expels,treats the stomach and opens the bowels,and also relieves pain.From the perspective of Sanwan acupoint corresponding to Three-Jiao,we further analyze the difference between the rise and fall of Qi and Blood of the three acupoints.The deep layers of the three points are important organs.Before clinical acupuncture,careful physical examination should be performed,and appropriate acupuncture methods should be adopted according to individual conditions.The six hollow viscera must keep its unobstructed,therefore,points are selected with reducing manipulation in most cases.Based on the above perspectives,we interpret and analyze Sanwan acupoints in order to further explore the clinical value of the three acupoints.
