摘 要:目的探讨伴明显肝性脑病(OHE)的肝硬化病人静息态脑功能网络拓扑属性的变化特征。方法前瞻性纳入肝硬化病人57例(伴OHE 24例,无OHE 33例)和性别、年龄及受教育程度相匹配的正常对照组(HC)40例。所有受试者均行静息态功能MRI扫描,采用GRETNA软件对数据进行预处理,采用自动解剖标记(AAL)模板提取每个被试90个脑区的平均时间信号序列并基于图论的方法构建功能网络,采用单因素方差分析比较脑网络全局属性及局部属性各指标间的差异并采用Bonferroni多重比较校正。采用Pearson相关性分析观察全脑网络指标与认知评分之间的相关性。结果全局属性方面,OHE组聚类系数较HC组显著降低(P<0.05)。局部属性方面,与HC组相比,OHE组左侧中扣带/副扣带回、右侧颞中回的度中心性(DC),右侧颞中回的节点效率(NE)明显减低;左侧额中回的DC、左侧额中回及双侧尾状核的NE升高;与无OHE组相比,OHE组右侧颞中回的DC及NE明显降低,左侧额中回的DC及NE明显增高。OHE组左侧额中回的DC改变与认知评分DST具有相关性(r=0.42,P=0.03)。结论OHE病人静息态脑功能网络具有小世界属性,发生的全局属性及部分属性改变主要影响大脑DMN网络,这可能是OHE导致肝硬化病人认知障碍的原因之一。Objective To investigate the characteristics of topological properties of resting brain functional network in patients with overt hepatic encephalopathy(OHE).Methods A total of 57 cirrhotic patients(24 OHE patients and 33 no-OHE patients)and 40 gender,age and education matched normal controls(HC)were prospectively included.All the patients underwent magnetic resonance resting state functional MRI examination,and the data was preprocessed by GRETNA software.Then average temporal signal series of 90 brain regions according to AAL template were extracted to construct functional network.The global and local topological properties were analyzed and compared based on graph theory.One-way ANOVA was used to compare differences in indices for global and local properties of the brain network between the three groups and used Bonferroni’s correction for multiple comparisons.Pearson correlation analysis was used to observe the correlation between topological properties and cognitive metrics.Results In terms of global properties,compared with HC group,the clustering coefficient of the OHE group showed a significant decrease.In terms of local properties,compared with HC group,the degree centrality of the left median cingulate/paracingulate gyri and the right middle temporal gyrus,and the nodal efficiency of the right middle temporal gyrus were significantly decreased,while the degree centrality of the left middle frontal gyrus,and the nodal efficiency of the left middle frontal gyrus and bilateral caudate nucleus were significantly increased in the OHE group.Compared with the no-OHE group,the degree centrality and nodal efficiency of the right middle temporal gyrus were significantly decreased in the OHE group,and the degree centrality and nodal efficiency of the left middle frontal gyrus were significantly increased.The correlation analysis showed that the degree centrality change of the left middle frontal gyrus correlated with the cognitive score of DST in the OHE group(r=0.42,P=0.03).Conclusion The restingstate brain fun
