职业倦怠护士大脑形态学结构改变的初步探索

摘　　要：目的探究职业倦怠护士是否存在大脑形态学结构的异常改变,并分析其与职业倦怠量表(Maslach Burnout Inventory,MBI)各维度评分的相关性。方法根据MBI评分将2020年8月~12月在甘肃省某综合性三甲医院工作的30名护士纳入研究作为职业倦怠组,同时选择一般资料相匹配的29名无倦怠护士作为对照组。对两组护士进行静息状态下磁共振成像(magnetic resonance imaging,MRI)扫描,使用基于体素的形态学分析方法(voxel-based morphometry,VBM)分析被试脑灰质体积发生改变的脑区。结果与对照组相比,职业倦怠组护士灰质体积增大的脑区为左侧眶部额上回与额中回和右侧额中回;灰质体积减小的脑区为右侧小脑1区、2区和8区。Pearson相关分析显示,职业倦怠组灰质体积改变脑区与护士MBI各维度评分之间均无显著相关性(P>0.05)。结论职业倦怠护士部分脑区灰质体积发生改变,提示职业倦怠护士可能存在大脑皮层功能区的萎缩或增生。Objective To explore whether there are abnormal changes in brain morphological structure in burnout nurses,and to analyze its correlation with burnout scale scores.Methods According to the occupational burnout scale(MBI)score,30 nurses who worked in a comprehensive tertiary hospital in Gansu province from August to December 2020 were included in the study as the occupational burnout group,and 29 non-burnout nurses with matched general data were selected as the control group.The two groups of nurses were scanned by magnetic resonance imaging(MRI)in resting state,based on VBM analysis method was used to measure brain areas with altered grey matter volume.Results Compared with the control group,the brain areas with increased gray matter volume in the burnout group included the left upper orbital frontal gyrus and middle frontal gyrus,and the right middle frontal gyrus.The brain areas with reduced gray matter volume were on the right Cerebellum 1,2,and 8 areas.Pearson correlation analysis showed that there was no significant correlation between the changes of gray matter volume in the burnout group and the scores of each dimension of the Nurse Burnout Scale(P>0.05).Conclusion The gray matter volume of some brain areas of burnout nurses changed,suggesting that burnout nurses may have atrophy or hyperplasia of cerebral cortex function area.
