摘  要:目的 追踪观察囊谦县和贵南县在实施碘缺乏病干预措施9年后,8~10岁在校儿童的碘营养和智力发展状况,以评估补碘干预措施对保护儿童智力发展的影响。方法 采用中国联合型瑞文测验(农村版)第二次修订版评估儿童IQ,并对儿童IQ进行校正[利用弗林效应(FE)增益量],校正IQ值=(IQ起点值-IQ终点值)-FE增益量(0.74点/年计算);对比补碘前(2012年)、补碘后(2021年)2县盐碘和尿碘指标、儿童校正IQ值以及不同年龄儿童IQ之间的发展差异。结果 碘盐、尿碘指标方面,补碘9年后囊谦县达到碘缺乏病消除标准;IQ指标方面,经过FE增益量校正后,囊谦县和贵南县补碘后儿童IQ值较补碘前实际增益量分别为10.34点、2.34点;2县补碘前儿童IQ值差异有统计学意义(Z=-6.181,P<0.05),补碘后儿童IQ值差异无统计学意义(Z=-0.196,P>0.05)。年龄方面,补碘后囊谦县8岁组与10岁组之间IQ值差异有统计学意义(P<0.05)。2县之间比较,补碘前8岁组、9岁组、10岁组儿童IQ值差异有统计学意义(Z=-2.606,-4.155,-3.929,P<0.05);补碘后8岁组、9岁组、10岁组儿童IQ值差异无统计学意义(Z=-1.401,-0.689,-0.901,P>0.05)。结论 实施补碘措施后对囊谦县儿童智力发育起到了显著的促进作用,普遍食盐加碘对持续消除碘缺乏危害、提高人群整体素质具有实际意义。Objective The iodine nutrition and intellectual development of school children aged 8-10 years after the implementation of iodine deficiency disorders(IDD) intervention in Nangqian County and Guinan County for 9 years were followed to evaluate the effect of iodine supplement intervention on the protection of children’s intellectual development.Methods Children’s IQ was assessed using the second revision of China Joint Raven Test(rural version),and the children’s IQ was corrected [using Flynn effect(FE) gain].The corrected IQ value was equal to(the starting point of IQ-the end point of IQ)-FE gain(0.74 points per year).The indicators of salt iodine and urinary iodine, adjusted IQ of children and developmental differences of IQ of children at different ages were compared between the two counties before iodine supplementation(2012) and after iodine supplementation(2021).Results In terms of iodized salt and urine iodine, Nangqian County reached the standard of eliminating IDD after 9 years of iodine supplement.In terms of IQ index, after FE gain correction, the IQ of children in Nangqian County and Guinan County after iodine supplement was 10.34 points and 2.34 points respectively compared with the actual gain before iodine supplement.There was significant difference in IQ before iodine supplement(Z=-6.181,P<0.05),but no significant difference in IQ after iodine supplement(Z=-0.196,P>0.05).In terms of age, there was a statistically significant difference in IQ between the 8 years old group and the 10 years old group in Nangqian County after iodine supplementation(P<0.05).Compared between the two counties, there were significant differences in the IQ values of children aged 8,9 and 10 years before iodine supplementation(Z=-2.606,-4.155,-3.929,P<0.05).There was no significant difference in IQ of children aged 8,9 and 10 years after iodine supplementation(Z=-1.401,-0.689,-0.901,P>0.05).Conclusion The implementation of iodine supplement has played a significant role in promoting the intellectual development of chil
