摘  要:目的 探讨甲状腺乳头状癌组织中VDR、BRD4、HOXB9的表达及其临床意义。方法 收集甲状腺乳头状癌患者150例,留取肿瘤组织以及癌旁组织,采用免疫组织化学染色法检测组织中VDR、BRD4、HOXB9的表达,并分析VDR、BRD4、HOXB9表达与甲状腺乳头状癌临床病理因素的关系,Pearson相关性分析VDR、BRD4、HOXB9蛋白的相关性。结果 甲状腺乳头状癌组织中VDR、BRD4阳性表达率(65.33%、65.33%)均显著高于癌旁组织(χ^(2)=89.955,P=0.000;χ^(2)=57.252,P=0.000),甲状腺乳头状癌组织中HOXB9阳性表达率(34.67%)显著低于癌旁组织(χ^(2)=77.912,P=0.000)。VDR阳性表达率在瘤体大小>1 cm、瘤体内钙化以及TNM分期为Ⅲ+Ⅳ期患者中明显更高(P<0.05);BRD4阳性表达率在瘤体大小>1 cm、淋巴结转移、TNM分期为Ⅲ+Ⅳ期患者中明显更高(P<0.05);HOXB9阳性表达率在瘤体大小>1 cm、淋巴结转移、TNM分期为Ⅲ+Ⅳ期患者中明显更低(P<0.05)。HOXB9蛋白表达分别与VDR及BRD4蛋白表达呈负相关(γ=-0.291,γ=-0.412,P均<0.05),VDR蛋白表达与BRD4蛋白表达呈正相关(γ=0.406,P<0.05)。结论 VDR、BRD4在甲状腺乳头状癌中均呈现高表达,HOXB9在甲状腺乳头状癌中均呈现低表达,VDR、BRD4、HOXB9表达均与甲状腺乳头状癌发生和发展有关,对临床诊断及治疗均具有指导意义。Objective To investigate the expression of VDR,BRD4 and HOXB9 in papillary thyroid carcinoma and their clinical significance.Methods 150 patients with thyroid papillary carcinoma were collected,tumor tissues and adjacent tissues were collected,and the expressions of VDR,BRD4 and HOXB9 in the tissues were detected by immunohistochemical staining,and the relationship between the expressions of VDR,BRD4 and HOXB9 and clinicopathologic factors of thyroid papillary carcinoma was analyzed.The correlations of VDR,BRD4 and HOXB9 proteins were analyzed by Pearson correlation.Results The positive expression rates of VDR and BRD4 in papillary thyroid carcinoma tissues(65.33%,65.33%)were significantly higher than those in paracancer tissues(χ^(2)=89.955,P=0.000;χ^(2)=57.252,P=0.000),the HOXB9 positive expression rate in papillary thyroid carcinoma tissues(34.67%)was significantly lower than that in paracancer tissues(χ^(2)=77.912,P=0.000).The VDR positive expression rate was significantly higher in patients with tumor size>1 cm,intratumor calcification and TNM stageⅢ+Ⅳ(P<0.05).The positive expression rate of BRD4 was significantly higher in patients with tumor size>1 cm,lymph node metastasis and TNM stageⅢ+Ⅳ(P<0.05).The HOXB9 positive expression rate was significantly lower in patients with tumor size>1 cm,lymph node metastasis and TNM stageⅢ+Ⅳ(P<0.05).HOXB9 protein expression was negatively correlated with VDR and BRD4 protein expression(γ=-0.291,γ=-0.412,P<0.05),and VDR protein expression was positively correlated with BRD4 protein expression(γ=0.406,P<0.05).Conclusion VDR and BRD4 are highly expressed in thyroid papillary carcinoma,while HOXB9 is low expressed in thyroid papillary carcinoma.The expression of VDR,BRD4 and HOXB9 is related to the occurrence and development of thyroid papillary carcinoma,which has guiding significance for clinical diagnosis and treatment.
