结直肠癌中PD-L1的表达及其与临床病理特征的关系

摘　　要：目的观察结直肠癌中程序性细胞死亡配体(PD-L1)的表达并分析其与临床病理特征的关系。方法收集潍坊市中医院2018年1月~2022年3月期间根治性手术治疗的结直肠癌患者193例,采用免疫组化方法检测PD-L1在癌组织及癌旁组织的表达。结果结直肠癌组织中PD-L1蛋白阳性表达率为27.98%(54/193),明显高于癌旁组织3.11%(6/193);结直肠癌组织中PD-L1表达与病理分型(P=0.002)、TNM分期(P<0.001)、肿瘤大小(P=0.022)和淋巴结转移(P<0.001)具有相关性,差异具有统计学意义;与年龄(P=0.124)、性别(P=0.949)、部位(P=0.119)无关,差异无统计学意义;在淋巴结转移灶中PD-L1的表达与转移癌分化程度具有相关性(P=0.031),与转移灶的大小无关(P=0.672)。结论PD-L1在结直肠癌中表达率较癌旁组织有所升高,但与其他肿瘤相比表达率低,其表达与患者的临床病理特征密切相关。PD-L1在淋巴结转移癌中的表达率下降。PD-L1的表达对结直肠癌诊断和治疗有重要临床价值。Objective To investigate the expression of PD-L1 in colorectal cancer and its clinicopathologic features.Methods From January 2018 to March 2022,a total of 193 patients with colorectal cancer underwent surgery in Weifang Traditional Chinese Medicine Hospital were enrolled in the study.The expression of PD-L1 protein was detected by immunohistochemistry.Results The positive rate of PD-L1 protein expression was 27.98%(54/193).The expression of PD-L1 in colorectal cancer was significantly higher than that in para-cancerous tissues 3.11%(6/193).PD-L1 was correlated with pathological classification(P=0.002),TNM stage(P0.001),tumor size(P=0.022)and lymph node metastasis(P0.001).There was no significant difference among age(P=0.124),sex(P=0.949)and parts(P=0.119).The expression of PD-L1 in lymph node metastases was related to the differentiation(P=0.031),but independent of the size(P=0.672).Conclusion The expression of PD-L1 in colorectal cancer is higher than that in adjacent tissues.But the expression of PD-L1 in colorectal cancer is lower than that in other tumors.The expression of PD-L1 is closely related to the clinicopathological characteristics of colorectal cancer,and plays an important role in the diagnosis and treatment of colorectal cancer.
