摘 要:目的探讨使用新媒体平台中的互动视频作为基于案例的教学法(case-based learning,CBL)的教学手段培养超声住院医师规范化培训(简称住培)学员综合临床能力及临床思维的方法及应用效果。方法选取2019级及2020级东南大学附属中大医院超声医学科的16名住培学员,比较接受基于网络视频平台的CBL学习或传统CBL学习的两组学员参与度、自身满意度、理论知识及实践操作水平。结果接受基于网络视频平台的CBL学习的学员较接受传统CBL学习的学员对教学方法激发学习兴趣[9.5(9.0,10.0)vs.7.0(5.5,7.0),P=0.001],提升理论水平[9.0(8.0,9.0)vs.7.0(7.0,7.8),P=0.002]、操作水平[8.0(7.3,9.0)vs.3.0(3.0,4.4),P=0.001]、医学素养[10.0(8.3,10.0)vs.8.0(7.3,9.0),P=0.019]及临床思维[10.0(8.3,10.0)vs.8.5(7.3,9.0),P=0.029]的满意度更高,且实操考核成绩[95.0(86.3,95.0)vs.82.5(80.0,90.0),P=0.040]更高,差异有统计学意义(P<0.05)。结论基于网络视频平台的CBL学习较传统CBL学习更能激发学员的学习兴趣,且对于提高学员理论水平、实操技巧及临床思维能力具有更好的效果。Objective To explore the method and application effect of using interactive video on new media platform as case-based learning(CBL)teaching method to cultivate the comprehensive clinical ability and clinical thinking of ultrasonic resident trainees.Methods A total of 16 resident trainees from the department of ultrasound medicine of Zhongda Hospital,Southeast University in 2019 and 2020 were selected to compare the participation,satisfaction,theoretical knowledge and practical operation level of the two groups of trainees who received CBL learning based on network video platform or traditional CBL learning.Results Students who accept CBL learning based on network video platform are more excited about teaching methods than those who accept traditional CBL learning interest[9.5(9.0,10.0)vs.7.0(5.5,7.0),P=0.001],improving theoretical level[9.0(8.0,9.0)vs.7.0(7.0,7.8),P=0.002],operational level[8.0(7.3,9.0)vs.3.0(3.0,4.4),P=0.001],medical literacy[10.0(8.3,10.0)vs.8.0(7.3,9.0),P=0.019]and clinical thinking[10.0(8.3,10.0)vs.8.5(7.3,9.0),P=0.029]was higher,and the practical performance test score[95.0(86.3,95.0)vs.82.5(80.0,90.0),P=0.040]was higher,and the differences were statistically significant(P<0.05).Conclusion Compared with traditional CBL learning,CBL learning based on network video platform can stimulate students'interest in learning,and has a better effect on improving students'theoretical level,practical skills and clinical thinking ability.
