摘  要:目的探讨迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)联合操作技能直接观察评估(direct observation of procedural skill,DOPS)在心血管内科实习医生临床教学中的应用效果。方法选取2018年6月—2021年6月在南京医科大学附属淮安第一医院心血管内科实习的临床本科实习医生100名为研究对象,根据随机数表法分为研究组(在传统教学基础上,第2周进行1次Mini-CEX联合DOPS教学)和对照组(接受传统教学),每组50名。两组实习医生出科时进行Mini-CEX、DOPS测评、出科理论考核,通过教学满意度问卷调查来评价教学效果。结果研究组出科时问诊技巧、体格检查、人文关怀、临床判断、沟通技能、组织效能和整体临床胜任力方面的Mini-CEX量表评分均高于对照组,差异有统计学意义(P<0.05);研究组DOPS评分高于对照组,差异有统计学意义(P<0.05)。出科时两组实习医生理论成绩比较,差异无统计学意义(P>0.05)。研究组实习医生和临床带教老师的双向满意度均高于对照组,差异有统计学意义(P<0.05)。结论Mini-CEX联合DOPS教学能有效提高心血管内科实习医生的临床综合能力和操作技能水平。Objective To explore the application effect of mini-clinical evaluation exercise(Mini-CEX)combined with direct observation of procedural skill(DOPS)in clinical teaching of cardiovascular interns.Methods A total of 100 clinical undergraduate interns who were interns in the Department of Cardiovascular Medicine,Huai´an First Hospital,Affiliated to Nanjing Medical University,from June 2018 to June 2021 were selected as the study subjects.They were divided into a study group(based on traditional teaching and conducted Mini-CEX combined with DOPS teaching once in the second week)and a control group(receiving traditional teaching)according to a random number table method,with 50 in each group.The two groups of interns were evaluated by Mini-CEX,DOPS and theoretical assessment when they left the department.The teaching effect was evaluated through a questionnaire survey of teaching satisfaction.Results The study group had higher scores on the Mini-CEX scale in terms of consultation skills,physical examination,humanistic care,clinical judgment,communication skills,organizational effectiveness,and overall clinical competence at the time of discharge than the control group,and the difference was statistically significant(P<0.05).The DOPS score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the theoretical scores of the two groups of interns at the time of graduation(P>0.05).The two-way satisfaction of interns and clinical teaching teachers in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Mini-CEX combined with DOPS teaching can effectively improve the clinical comprehensive ability and operational skills of practicing physicians in cardiovascular medicine.
