摘 要:目的探讨基底动脉瘤的血液流变。方法于2018年1月至2020年1月在牡丹江医学院附属红旗医院放射科进行原始影像数据采集,以基底动脉磁共振血管成像图像为基础构建基底动脉三维几何模型,动脉瘤组与对照组各20例,应用计算流体力学方法对血管进行数值模拟分析。结果获得了基底动脉瘤模型的血流动力学参数,数值模拟结果血流速度、壁面压力(WP)及震荡剪切系数(OSI)三项指标,动脉瘤组其数值均高于对照组,差异有统计学意义(P<0.05)。动脉瘤颈处的血流速度大于瘤腔内的血流速度,表现为高速血流向低速涡流转变,血流主要撞击点为左侧瘤壁。动脉瘤组高WP主要分布在双侧椎动脉,瘤体整体为均匀稍高WP。动脉瘤组高OSI集中分布瘤颈部及左侧瘤体部,并且OSI不稳定,与低速涡流血管区域重合。结论基底动脉瘤血流不稳定,表现为高WP及波动OSI,以瘤颈及左侧瘤壁区域明显,血管壁损伤概率较大,瘤体破裂风险增加,临床应着重关注该区域,进行个体化治疗。本研究为基底动脉瘤等心脑血管疾病提供理论依据,并为动脉瘤血管支架手术提供血流动力学支持。Objective To investigate the hemorheology of basilar artery aneurysm.Methods The original image data in the Department of Radiology of Hongqi Hospital Affiliated to Mudanjiang Medical University from January 2018 to January 2020 were collected.Based on the magnetic resonance angiography images of basilar artery,three-dimensional geometric models of basilar artery were constructed,with the two groups of the aneurysm group(n=20)and the control group(n=20)obtained according to the images.The blood vessels were numerically simulated and analyzed by computational fluid dynamics.Results The hemodynamic parameters of the basilar artery aneurysm model were obtained.The numerical simulation results showed that the values of three indices,namely the blood flow velocity,wall pressure(WP)and oscillatory shear stress(OSI),and the values of the aneurysm group were all higher than those of the control group,with statistically significant differences(P<0.05).The blood flow velocity at the neck of the aneurysm was greater than that in the tumor cavity,which showed the transition from high-speed blood flow to low-speed vortex,and the main impact point of blood flow was the left tumor wall.In the aneurysm group,high WP was mainly distributed in bilateral vertebral arteries,and the whole tumor body had uniform and slightly higher WP.In the aneurysm group,the high OSI was concentrated at the neck and in the left tumor body,and the OSI was unstable,which coincided with the low-speed vortex vascular area.Conclusion The blood flow of basilar artery aneurysm is unstable,showing high WP and fluctuating OSI,especially at the neck and in the left tumor wall area.There is a higher probability of vascular wall injury and an increased risk of tumor rupture.Clinically,we shall pay attention to this area and carry out individualized treatment.This research provides theoretical basis for cardiovascular and cerebrovascular diseases such as basilar artery aneurysm and provides hemodynamic support for aneurysm stent surgery.
