中国老年肿瘤患者衰弱的现状及影响因素分析

摘　　要：目的调查老年肿瘤患者衰弱的现状及影响因素。方法采用便利抽样的方法抽取2022年2月至2022年5月间蚌埠医学院第一附属医院肿瘤病区的198例老年肿瘤患者为研究对象,应用老年肿瘤患者社会学人口学资料问卷、疾病相关资料问卷、衰弱表型量表及日常生活能力量表进行调查;采用单因素及有序Logistic回归分析老年肿瘤患者衰弱现状及影响因素。结果老年肿瘤患者衰弱分范围为1~5分,中位数为3(1,4),其中无衰弱19例(9.60%),衰弱前期97例(48.99%),衰弱期82例(41.41%)。有序Logistic回归分析显示:年龄、日常生活活动能力对老年肿瘤患者衰弱得分的影响有统计学意义(P<0.05)。结论老年肿瘤患者衰弱发生率较高且受年龄及日常生活活动能力的影响,医务人员通过识别其衰弱程度,分析其影响因素,为患者提供最佳的治疗和护理方案,延长老年肿瘤患者预期寿命,提高其生活质量。Objective To investigate the status and influencing factors of senile tumour patients with frailty.Methods 198 elderly cancer patients in the tumour ward of the First Affiliated Hospital of Bengbu Medical College from February 2022 to May 2022 were selected by convenient sampling method as the research objects.The elderly cancer patients’sociological,demographic data questionnaire,disease history,frailty phenotype scale and ability of daily living scale were used for the questionnaire survey;Single factor and multiple regression analyses were used to analyze the status and influencing factors of elderly cancer patients.Results The total frailty score of elderly cancer patients ranged from 1 to 5 points,with a median of 3(1,4),including 19 patients without frailty symptoms(9.6%),97 patients with pre-frailty symptoms(48.99%),and 82 patients with frailty symptoms(41.41%).Ordered logistic regression analysis showed that age and activities of daily living had a statistically significant impact on frailty scores in elderly cancer patients(P<0.05).Conclusion The incidence of senility in tumour patients is high and is affected by many factors.Medical personnel need to judge the degree of senility of tumour patients and identify the influencing factors which provide the best treatment and nursing plan for patients,prolong the life expectancy of elderly tumour patients,and improve their quality of life.
