摘 要:目的探讨上皮性卵巢癌(EOC)组织长链非编码RNA(lncRNA)锌指蛋白反义链1(ZFAS1)、Notch1的表达水平对患者预后的评估价值。方法选取本院2018年1月~2020年1月收治的96例EOC患者为EOC组,将同期卵巢良性肿瘤患者96例作为对照组。使用实时荧光定量PCR法测定所有研究者卵巢组织中ZFAS1、Notch1的表达水平;ROC曲线评估ZFAS1、Notch1诊断EOC的价值,并分析ZFAS1、Notch1表达与EOC患者临床病理特征的关系;使用Kaplan-Meier法绘制EOC患者生存曲线;使用多因素Cox回归分析EOC患者预后的影响因素。结果EOC组组织中ZFAS1、Notch1表达水平高于对照组,差异有统计学意义(P<0.05);ZFAS1单独诊断EOC曲线下面积为0.861(95%CI:0.803~0.919),截断值为1.59,灵敏度为77.1%,特异度为69.8%;Notch1单独诊断EOC的曲线下面积为0.913(95%CI:0.870~0.955),截断值为1.54,灵敏度为79.2%,特异度为76.1%;ZFAS1表达与EOC患者的FIGO分期、浸润深度、淋巴结转移、病理分级有关(P<0.05);Notch1表达与EOC患者的FIGO分期、淋巴结转移、病理分级有关(P<0.05);ZFAS1高表达患者(ZFAS1≥1.59)的3年累积生存率低于低表达患者(ZFAS1<1.59),Notch1高表达患者(Notch1≥1.54)的3年累积生存率低于低表达患者(Notch1<1.54),差异均有统计学意义(P<0.05);多因素Cox回归模型分析表明,FIGO分期、淋巴结转移、ZFAS1、Notch1是影响EOC患者预后的独立危险因素(P<0.05)。结论EOC患者癌组织中ZFAS1、Notch1表达水平升高,且ZFAS1、Notch1高表达的EOC患者生存期短,两者有望成为预测EOC预后的有效生物学标志物。Objective To assess the prognostic significance of the expression levels of two specific long non-coding RNA(lncRNA),namely zinc finger protein antisense chain 1(ZFAS1)and Notch1,in epithelial ovarian carcinoma(EOC)tissue.Methods A total of 96 patients diagnosed with EOC and admitted to Handan First Hospital between January 2018 and January 2020 were included in the EOC group.In parallel,another 96 patients with benign ovarian tumors during the same period were selected as the control group.The expression levels of ZFAS1 and Notch1,were quantified using real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Receiver operating characteristic(ROC)curve analysis was performed to assess the diagnostic value of ZFAS1 and Notch1 in EOC.Furthermore,the relationship between the expression of ZFAS1 and Notch1 and various clinicopathologic features was examined.Survival analysis was conducted using the Kaplan-Meier method,and multivariate Cox regression was employed to analyze prognostic factors.Results The expression levels of ZFAS1 and Notch1 were found to be significantly higher in the EOC group compared to the control group(P0.05).ZFAS1 alone demonstrated an area under the curve(AUC)of 0.861(95%CI:0.803-0.919)for diagnosing EOC,with a cut-off value of 1.59,sensitivity of 77.1%,and specificity of 69.8%.Similarly,Notch1 alone showed an AUC of 0.913(95%CI:0.870-0.955)for diagnosing EOC,with a cut-off value of 1.54,sensitivity of 79.2%,and specificity of 76.1%.The expression of ZFAS1 was significantly correlated with FIGO stage,invasion depth,lymph node metastasis,and pathological grade(P<0.05),while Notch1 expression was significantly associated with FIGO stage,lymph node metastasis,and pathological grade(P<0.05).Patients with high expression of ZFAS1(ZFAS1≥1.59)and Notch1(Notch1≥1.54)had significantly lower 3-year cumulative survival rates compared to those with low expression(P<0.05).Furthermore,multivariate Cox regression analysis revealed that FIGO stage,lymph node metastasis,ZFAS1,and Notch1 were
