摘　　要：目的构建一种安全、有效且易推广的腹腔镜技术培训方法,可用于腹腔镜腹股沟疝修补术(laparoscopic inguinal hernia repair,LIHR)的教学。方法对2018年9月至2021年7月20位进入复旦大学附属中山医院普外科疝与腹壁外科组培训的普通外科专科医师规范化培训(简称专培)医师进行LIHR的模块化培训。按难易程度及危险程度将LIHR分为5个模块,由该组指导医师按评分标准对专培医师的操作技术进行评估、考核,并分析模块化培训系统对腹腔镜手术技术培训的作用。结果20名参与模块化培训的普通外科专培医师中,6名(30%)完成了全程5个模块的培训,3名(15%)完成了前4个模块的培训,7名(35%)完成了前3个模块的培训。专培医师参加的手术和完成全部5个模块培训后独立完成的手术均无疝复发和也无Trocar疝发生。结论LIHR模块化培训方法在普外科专科医师培训中安全、有效,可以用于腹腔镜技术培训和LIHR教学,且易推广。Objective To construct a safe,effective and easy way to popularize the laparoscopic technique training method,which can be used in laparoscopic inguinal hernia repair(LIHR)teaching.Methods From September 2018 to July 2021,20 general specialists who entered the professional group of hernia and abdominal wall surgery in Zhongshan Hospital,Fudan University were trained by LIHR STEP by STEP modular training.LIHR was divided into five modules according to the degree of difficulty and risk.Guide teachers in this professional group evaluated and examined the specialists'operation skills according to detailed scoring standards.The effect of STEP by STEP modular training system on laparoscopic surgery training was analyzed and evaluated.Results Among the 20 general specialists who participated in STEP by STEP modular training,6(30%)completed the training of five modules,3(15%)completed the first four modules and 7(35%)completed the first three modules.There was no recurrence of hernia and no Trocar hernia in the operations attended by the trainees,and the operations completed independently after completing all five modules of training.Conclusion LIHR modular training is safe and effective in general surgical specialist training.It can be used in laparoscopic training and LIHR teaching and is easy to popularize.
