摘  要:目的定量分析脊髓性肌萎缩症患儿大腿肌肉脂肪浸润程度,探讨肌肉脂肪分数(FF)值与临床运动功能的相关性。方法对20例SMA患儿进行Hammersmith运动功能量表(HFMSE)评分及大腿肌肉M-Dixon Quant成像,计算大腿13块肌肉的FF值。并采用Spearman秩相关分析评估不同肌肉FF值和临床功能相关性。结果臀大肌拥有最高的FF值,缝匠肌次之,长收肌拥有最低的FF值。肌肉平均FF值与HFMSE评分均呈负相关(r=0.699,P<0.05)。除长收肌(P>0.05)外,其余肌肉与HFMSE评分均呈负相关(P<0.05),其中以臀大肌相关性最高(r=0.789),半腱肌相关性最弱(r=0.512)。彩色编码的FF图提供了对肌肉脂肪含量及其异质性分布的有效视觉评估。结论M-Dixon Quant脂肪定量技术可客观定量评估脊髓性肌萎缩症患儿疾病的严重程度,臀大肌FF值测量可能是反映临床运动功能的理想指标。Objective To quantitatively analyze the degree of fat infiltration in thigh muscles of children with spinal muscular atrophy(SMA),and to explore the correlation between the mean fat fraction(FF)and clinical motor function.Methods Hammersmith Functional Motor Scale Expanded(HFMSE)and M-Dixon Quant imaging of thigh muscles were performed in 20 children with SMA.The FF values of 13 muscles of thigh were calculated.The correlation between the FF values of different muscles and clinical function was analyzed by Spearman rank correlation.Results The gluteus maximus had the highest FF value,followed by the sartorius muscle,and the adductor longus had the lowest FF value.The mean FF value of muscles was negatively correlated with HFMSE score(r=-0.699,P<0.05).All muscles were negatively correlated with HFMSE score(P<0.05),except for the adductor longus(P>0.05),FF value of the gluteus maximus had the highest correlation(r=0.789),which semitendinosus had the weakest correlation(r=0.512).Color-coded FF maps provide an effective visual assessment of muscle fat content and its heterogeneous distribution.Conclusio1nM-Dixon Quant fat quantification technology can objectively and quantitatively assess the severity of the disease in children with SMA,and the measurement of the FF value of the gluteus maximus may be an ideal indicator to reflect clinical motor function.
