摘  要:目的探讨TPH基因A218C、MAOA基因T1460C多态性与父母体罚在双相障碍冲动攻击行为的交互作用。方法回顾性选择2015年3月至2021年6月于广州中医药大学第二临床医学院门诊或住院治疗的BD患儿为研究对象,依据MOAS将其分为冲动攻击行为组和无冲动攻击行为组,采用双脱氧末端终止法检测基因多态性;多因素分析通过Logistic回归模型进行,并通过相加交互模型分析TPH A218C位点等位基因C、MAOA T1460C位点等位基因C和父母体罚对BD患儿冲动攻击行为的交互作用。结果TPH基因A218C位点C等位基因及CC基因型在发生冲动攻击行为的BD患者中占比显著高于无冲动攻击行为患者(P<0.05);MAOA基因T1460C位点C等位基因及CC基因型在发生冲动攻击行为的BD患者中占比显著高于无冲动攻击行为患者(P<0.05)。TPH基因A218C(CC/AC)和MAOA基因T1460C(CC/TC)及父母体罚均是BD发生冲动攻击行为的危险因素(P<0.05)。TPH基因A218C位点C等位基因、MAOA基因T1460C位点C等位基因及父母体罚在BD患者发生冲动攻击行为中具有两两交互作用。结论携带A218C(CC/AC)、T1460C(CC/TC)的个体属易发生冲动攻击行为人群,这些基因型和父母体罚的交互作用促进了BD患者冲动攻击行为的发生。Objective To investigate the interaction between TPH gene A218C,MAOA gene T1460C polymorphisms and parental corporal punishment on impulsive aggression in bipolar disorder.Methods A retrospective study was conducted on children with BD who were treated in the Second Clinical College of Guangzhou University of Chinese Medicine from March 2015 to June 2021.They were divided into impulsive aggressive behavior group and non impulsive aggressive behavior group according to the modified overt aggression scale(MOAS).The gene polymorphism was detected by dideoxy terminal termination method.Multivariate logistic regression model was used to analyze the risk factors of impulsive aggression in patients with BD,the interaction between TPH A218C allele C,MAOA T1460C allele C and parental corporal punishment on impulsive aggression in children with BD was analyzed by additive interaction model.Results The percentage of C allele and CC genotype of TPH gene A218C locus in patients with BD with impulsive aggression was significantly higher than that in patients without impulsive aggression(P<0.05).The proportion of MAOA gene T1460C allele C and CC genotype in patients with BD with impulsive aggressive behavior was significantly higher than that in patients without impulsive aggressive behavior(P<0.05).TPH gene A218C(CC/AC),MAOA gene T1460C(CC/TC)and parental corporal punishment were risk factors for impulsive aggression in BD(P<0.05).The C allele of A218C locus of TPH gene,T1460C allele of MAOA gene and parental corporal punishment have a twofold interaction in the impulsive aggression of patients with BD.Conclusion Individuals carrying A218C(CC/AC)and T1460C(CC/TC)are prone to impulsive aggression.The interaction between these genotypes and parental corporal punishment promotes the occurrence of impulsive aggression in patients with BD.
