摘  要:目的 评价情景模拟教学法在住院医师规范化培训术前谈话教学中的应用效果。方法 以腹腔穿刺术前谈话教学为研究内容,选取2019年7月—2020年7月于上海市第一人民医院住培的住院医师为研究对象,其中23名2019级住院医师作为对照组,实施传统的小讲课教学;27名2020级住院医师作为研究组,实施情景模拟教学。采用理论考试测评、修订版利物浦医师沟通能力评价量表(Liverpool communication skills assessment scale,LCSAS)及问卷调查的方式评价教学效果。结果 两组住院医师的理论考试成绩差异无统计学意义(P> 0.05),培训后两组住院医师LCSAS评分均高于培训前,且研究组培训后评分明显高于对照组,差异有统计学意义(P <0.05)。结论 与传统的小讲课教学方式相比,情景模拟教学法更能高效率、全方位、多层次地提高住院医师的医患沟通能力及临床胜任力,以沉浸式的学习方式,促进住院医师快速适应临床工作。Objective To evaluate the effectiveness of the situational simulation teaching method in preoperative communication for standardized training of residents.Methods The study was conducted by selecting residents who underwent residential training at Shanghai General Hospital from July 2019 to July 2020.The research content was the teaching of preoperative communication before abdominal paracentesis.A total of 23 residents who were admitted in 2019 were selected as the control group and were taught through traditional medical lectures.A total of 27 residents who were admitted in 2020 were selected as the research group and were taught using situational simulation teaching.The teaching effects were evaluated through theoretical examination,revised Liverpool physician communication ability evaluation scale(LCSAS),and a questionnaire.Results There was no significant difference in the theoretical test scores between the two groups(P>0.05).However,the LCSAS scores of both groups after training were higher than before training,and the scores of the research group after training were significantly higher than those of the control group(P<0.05).Conclusion The situational simulation teaching method is more efficient and multi-level compared to traditional medical lectures.It can improve the doctor-patient communication ability and clinical competence of residents more effectively,and can also help residents adapt to clinical work more quickly through immersive learning.
