牙周整复术辅助口腔修复对牙体缺损患者牙周功能、龈沟液金属基质蛋白酶-8及超敏 C 反应蛋白表达水平的影响

摘　　要：目的:探究牙周整复术辅助口腔修复对牙体缺损患者牙周功能、空腹龈沟液(GCF)金属基质蛋白酶-8(MMP-8)、超敏C反应蛋白(hs-CRP)表达水平的影响。方法:随机选取牙体缺损患者82例,分为观察组(40例)和对照组(42例)。对照组采用常规口腔修复治疗,观察组采用牙周整复术辅助口腔修复治疗。对比两组临床疗效、牙齿功能[咬合时间(OT)、左右侧咬合力平衡度(BFDB)、咬合力占最大咬合力的百分比(MABF/MMF)]、牙周功能[牙周附着水平(AL)、探针深度(PD)、探针出血百分率(BOP)、菌斑百分比(PLI)]及GCF局部炎症指标[MMP-8、hs-CRP以及白细胞介素-6(IL-6)]变化情况。结果:两组患者治疗总有效率对比无统计学差异(P>0.05)。治疗后观察组PD、PLI、AL及BOP均低于对照组(均P<0.05)。治疗后相较于对照组,观察组MABF/MMF较大、OT较短、BFDB较低(均P<0.05)。观察组GCF中MMP-8、hs-CRP以及IL-6表达水平低于对照组(均P<0.05)。结论:牙周整复术辅助口腔修复能够显著改善牙体缺损患者的牙齿咬合及牙周功能,同时能够减少GCF中MMP-8、hs-CRP的表达水平、改善局部炎症,牙周整复术辅助口腔修复对牙体缺损患者的口腔健康具有积极作用。Objective:To investigate the effects of periodontal prosthesis assisted oral repair on periodontal function and the levels of metallomatrix proteinase-8(MMP-8)and hypersensitive C-reactive protein(hs-CRP)in fasting gingival crevicular fluid(GCF)in patients with dental defects.Methods:A total of 82 patients with dental defects were selected and randomly divided into observation group(40 cases)and control group(42 cases).The control group was treated with conventional oral prosthesis,and the observation group was treated with periodontal prosthesis assisted oral prosthesis.The therapeutic effect,dental function[occlusal time(OT),balance degree of left and right bite force(BFDB),percentage of bite force in maximum bite force(MABF/MMF)],periodontal function[periodontal attachment level(AL),probe depth(PD),percentage of probe bleeding(BOP),percentage of plaque(PLI)]and local inflammation indicators in GCF[MMP-8,hs-CRP,Interleukin-6(IL-6)]of the two groups were compared.Results:There was no difference in total response rate between the two groups.After treatment,PD,PLI,AL and BOP in the observation group were lower than those in the control group(all P<0.05).After treatment,MABF/MMF was higher,OT was shorter,and BFDB was lower in the observation group than in the control group(all P<0.05).The expression levels of MMP-8,hs-CRP and IL-6 in GCF in the observation group were lower than those in the control group(all P<0.05).Conclusion:Periodontal prosthesis assisted oral repair can significantly improve the dental occlusion and periodontal function of patients with dental defects,and can reduce the levels of MMP-8 and hs-CRP in GCF,and improve local inflammation.Periodontal prosthesis assisted oral repair has a positive effect on the oral health of patients with dental defects.
