摘  要:本手术为达芬奇机器人辅助肝中叶切除术。首先沿镰状韧带离断肝实质至肝中静脉,离断过程中结扎IV段Glisson蒂;其次沿Laennec膜分离显露右前肝蒂,分离并结扎肝蒂后显示右前和右后叶之间的分界线,离断肝中静脉,并沿Laennec间隙全程显露肝右静脉,完整切除肝中叶。手术时长225 min,出血量30 ml,手术离断肝脏实质从左肝到右肝,从足侧至头侧,手术过程流畅,手术视野清晰稳定,解剖层面清晰,管道结构显露完整,残肝流人流出道保护完好,体现了达芬奇手术机器人系统在复杂肝切除手术中明显的技术优势。This is a Da Vinci robot-assisted mesohepatectomy.First,the liver parenchyma was dissected along the falciform ligament to the middle hepatic vein.During the dissociation,the Glissonean pedicle of segment IV was ligated.Then,the right anterior hepatic pedicle was ligated along the Laennec capsule to mark the cutting line between the right anterior and posterior lobe.Finally,mesohepatectomy was completely performed after the middle hepatic vein was transsected and the right hepatic vein was exposed along the Laennec capsule.The operation duration was 225 mins with the blood loss of 30 ml.The liver parenchyma was dissected from the left lobe to the right lobe and from the foot side to the head side.The operation process was smooth with the stable surgical vision,clear anatomical identification and well protection of the inflow and outlow tract of the residual liver,which reflected the obvious technical advantages of the Da Vinci surgical robot system in complex liver resection.
