胃癌组织中SPARC、VEGF及HER2的表达及其与临床病理特征和预后的关系

摘　　要：目的探讨胃癌组织富含半胱氨酸的酸性分泌蛋白(Secreted protein acidic and rich in cysteine,SPARC)、血管内皮生长因子(Vascular endothelial growth factor,VEGF)及人类表皮生长因子受体-2(Human epidermal growth factor receptor-2,HER2)蛋白的表达及其与临床病理特征和预后的关系。方法选取手术切除胃癌组织标本80例及癌旁正常胃黏膜,采用免疫组织化学(Immunohistochemistry,IHC)方法检测SPARC、VEGF及HER2蛋白表达情况,并用荧光原位杂交(Fluorescence in situ hybridization,FISH)技术检测HER-2的基因扩增状况,研究胃癌组织SPARC、VEGF及HER2的表达与临床病理特征的关系,并分析其对预后的影响。结果SPARC、VEGF、HER2在胃癌中的表达率分别为45/80(56.25%)、49/80(61.25%)、25/80(31.25%),均高于正常胃黏膜,差异具有统计学意义(P<0.05)。SPARC主要在癌组织的间质细胞中表达,且与癌的Lauren分型、淋巴结转移相关(P<0.05)。VEGF的表达与癌的分化程度、Lauren分型、淋巴结转移相关(P<0.05)。HER2的表达与癌的分化程度、浸润深度、淋巴结转移相关(P<0.05)。SPARC阳性表达者5年生存率高于阴性表达患者(P<0.05);VEGF阳性及HER2阳性表达者5年生存率均低于阴性表达患者(P<0.05)。结论SPARC、VEGF及HER2在胃癌中的表达均高于正常胃黏膜,与胃癌的分化程度、Lauren分型、浸润深度、淋巴结转移等临床病理特征以及患者预后密切相关。Objective To investigate the expression of secreted protein acidic and rich in cysteine(SPARC),Vascular endothelial growth factor protein(VEGF),and human epidermal growth factor receptor-2(HER2)in gastric carcinoma and their relationship with clinicopathological features and prognosis.Methods The expression of SPARC,VEGF,and HER2 in 80 gastric cancer specimens and correspondent normal gastric mucosa adjacent to the cancer were detected by immunohistochemistry(IHC),the gene amplification of HER2 in gastric carcinoma was detected by Fluorescence in situ hybridization(FISH),and the relationship between the expression of SPARC,VEGF and HER2 and clinicopathological features was analyzed.Results The expression rates of SPARC,VEGF,and HER2 in gastric carcinoma were 45/80(56.25%),49/80(61.25%),and 25/80(31.25%),respectively,which were significantly higher than those in normal gastric mucosa(P<0.05).SPARC was mainly expressed in interstitial parts of cancer tissues and correlated with Lauren classification,and lymph node metastasis(P<0.05).The expression of VEGF was correlated with the degree of differentiation,Lauren classification,and lymph node metastasis(P<0.05).The expression of Her2 was correlated with the degree of differentiation,depth of invasion and lymph node metastasis(P<0.05).The 5-year survival rate of patients with positive expression of SPARC was higher than that of patients with negative expression(P<0.05),and the 5-year survival rate of patients with positive expression of VEGF and HER2 was lower than that of patients with negative expression(P<0.05).Conclusion The expression of SPARC,VEGF,and HER2 in gastric carcinoma is higher than that in normal gastric mucosa,which is closely related to the clinicopathological features of gastric carcinoma,such as differentiation,Lauren classification,depth of invasion,lymph node metastasis,and prognosis of patients.
