摘 要:目的探究血红蛋白电泳在地中海贫血诊断中的价值。方法选取2021年6月至2022年4月于东莞市东南部中心医院就诊的80例确诊为地中海贫血的患者作为研究组(α地中海贫血46例,β地中海贫血34例),另选择80名均为同期行健康体检的志愿者作为对照组。所有参检者均行血红蛋白电泳检测以明确地中海贫血类型。以地中海贫血基因检测结果为“金标准”,比较两组的血红蛋白电泳检测结果[包括血红蛋白F(HbF)、血红蛋白A(HbA)、血红蛋白A2(HbA2)];比较HbF、HbA2、HbA单项及联合检测在地中海贫血诊断中的应用价值。结果α地中海贫血患者HbF、HbA2水平均低于β地中海贫血,HbA水平高于β地中海贫血,差异有统计学意义(P<0.05);α地中海贫血患者的HbF高于对照组,HbA2水平低于对照组;β地中海贫血HbF、HbA2水平高于对照组,HbA水平低于对照组,差异有统计学意义(P<0.05)。HbF+HbA2+HbA联合检测在α地中海贫血及β地中海贫血诊断中应用价值最高,且灵敏度和特异度均高于HbF、HbA2、HbA单项检测。结论血红蛋白电泳在地中海贫血诊断中应用价值较高,应用联合检测的方式可较好弥补单项检测的不足,以获得更加确切的诊断结果,为临床诊疗提供可靠参考。Objective To explore the value of hemoglobin electrophoresis in the diagnosis of Thalassemia.Methods A total of 80 patients with Thalassemia diagnosed in Dongguan Southeast Central Hospital from June 2021 to April 2022 were selected as the study group(46 cases ofαThalassemia,34 cases ofβThalassemia),and another 80 volunteers who had health examinations at the same time were selected as the control group.All participants were tested by hemoglobin electrophoresis.The results of hemoglobin electrophoresis(including hemoglobin F[HbF],hemoglobin A[HbA],and hemoglobin A2[HbA2])of the two groups were compared with the results of Thalassemia gene detection as the"gold standard";To compare the application value of single and combined detection of HbF,HbA2 and HbA in Thalassemia.Results The levels of HbF and HbA2 in patients withαThalassemia were lower thanβThalassemia,HbA level was higher thanβThalassemia,the differences were statistically significant(P<0.05).The HbF ofαThalassemia patients was higher than that of the control group,and the HbA2 level was lower than that of the control group,the differences were statistically significant(P<0.05).The levels of HbF and HbA2 inβThalassemia were higher than those in the control group,and the level of HbA was lower than that in the control group,with statistically significant differences(P<0.05).The diagnostic value of HbF+HbA2+HbA combined detection inαThalassemia andβThalassemia is the highest,and the sensitivity and specificity were higher than those of HbF,HbA2,HbA single detection.Conclusion Hemoglobin electrophoresis is of high value in the diagnosis of Thalassemia.The application of combined detection can better make up for the shortcomings of single detection,so as to obtain more accurate diagnostic results and provide reliable reference for clinical diagnosis and treatment.
