寒热往来症状反应探析

摘　　要：寒热往来为中医特色症状反应,但其在《中医诊断学》的内涵、临床意义界定存在模糊之处,其在中医诊断学和临床教材中的应用也有一定衔接问题。为辨析寒热往来及其相关症状术语,厘清寒热往来的内涵与临床意义,文章结合古今文献展开探讨,得出“寒热往来”在古医籍中常用于形容外感、内伤病中的寒热交替现象;疟疾所见寒热往来,则用模糊定量表述寒热程度的术语。就其发作形式而言,疟疾所见寒热往来,先寒战后高热,发有定时;少阳病所见寒热往来,并不特别强调寒、热出现的先后顺序,发无定时。临床上,寒热往来并非半表半里证或疟疾的特征性症状。以期通过以上讨论,为寒热往来及其相关症状的规范化使用提供参考。Alternate cold and heat is a distinctive traditional Chinese medicine(TCM)symptom response,while it's vague and inconsistent in terms of connotation and clinical significance in various TCM Diagnostics textbooks,and it's not consistently applied in various TCM clinical textbooks.In order to differentiate the term of alternate cold and heat'and relevant symptom terms,and clarify the connotation and clinical significance of alternate cold and heat',an analysis is carried out in this paper by integrating ideas from both ancient and modern books and papers.On basis of that,the following ideas are confrmed:'Alternate cold and heat'commonly refers to alternation of cold and heat seen in exterior or interior patterns;if similar alternation is seen in malaria,it's described with terms of fuzzy quantitive evaluation of the degree of cold and heat.In malaria,high fever follows severe chills with fixed time of attack;while in shaoyang disease,cold and heat occur without fixed sequence or time of attack;Alternate cold and heat is not solely seen in half-exterior half-interior pattern or malaria.With such a differentiation and analysis,we hope to provide references for the standardized usage of relevant symptoms.
