工作坊模式住院医师规范化培训腰椎穿刺的教学改良分析

摘　　要：目的探讨工作坊模式住院医师规范化培训腰椎穿刺的教学改良效果。方法选择2018年4月—2020年4月南平市第一医院70名住院医师作为研究对象,根据时间点不同分为对照组(n=35,2018年4月—2019年4月末)和观察组(n=35,2019年5月—2020年4月末)。2组均完成腰椎穿刺教学,对照组采用传统方法教学,观察组采用工作坊下改良教学,2组均教学2周,比较2组住院医师考核合格率、满意度及腰椎穿刺术考核成绩。结果带教2周后观察组理论测试、腰椎穿刺技术、神经系统查体及腰椎穿刺注意事项合格率均高于对照组(P<0.05);带教2周后观察组教学方法、教学效果、教学内容、教学质量及教学形式满意度均高于对照组(P<0.05);带教2周后,相比对照组,观察组操作步骤与方法、操作规范以及熟练度成绩均更高(P<0.05),2组住院医师准备工作和操作后处理成绩比较差异均无统计学意义(P>0.05)。结论工作坊下改良教学用于住院医师规范化培训腰椎穿刺教学中能提高考核合格率及满意度,住院医师操作步骤与方法、操作规范和熟练度成绩均有所提升,值得推广应用。Objective To explore the standardized teaching effect of lumbar puncture workshop.Methods 70 residents from April 2018 to April 2020 were selected and divided into control group(n=35,April 2018 to the end of April 2019)and observation group(n=35,may 2019 to the end of April 2020).Both groups completed the teaching of lumbar puncture,the control group used the traditional teaching method,and the observation group improved the teaching based on the workshop on the basis of the control group.Both groups completed the teaching for two weeks.The qualified rate,satisfaction and teaching experience of residents in the two groups were compared.Results Two weeks after teaching,the qualified rates of theoretical test,lumbar puncture technology,nervous system physical examination and precautions of lumbar puncture in the observation group were higher than those in the control group(P<0.05).The teaching content,teaching method and teaching effect in the observation group were higher than those in the control group(P<0.05).Compared with the control group,the operation steps and methods,operation specifications and proficiency scores of the observation group were higher(P<0.05).There was no significant difference between the two groups in the scores of resident preparation and post-operation treatment(P>0.05).Conclusion The improved teaching under the workshop can improve the examination qualification rate and satisfaction in the standardized training of residents,and the operation steps and methods,operation specifications and proficiency of residents are improved,which is worthy of popularization and application.
