摘  要:目的对2021年重庆市万州区两起食物中毒事件分离的副溶血性弧菌进行分子溯源分析,并分析其毒力基因、耐药性,为副溶血性弧菌引起的食源性疾病的防控和临床治疗提供参考。方法采用全自动微生物质谱仪鉴定病原菌,Illumina测序平台进行全基因组测序,利用多位点序列分型和全基因组单核苷酸多态性(whole genome single nucleotide polymorphism,wgSNP)进行分子分型分析,数据库比对分析毒力和耐药基因,采用微量肉汤稀释法进行药敏实验。结果共鉴定8株副溶血性弧菌,其中7株来自患者,1株来自餐具。4株来自第1起食物中毒事件的副溶血性弧菌属于ST2156型克隆,wgSNP差异数为0,判断为同一克隆;3株来自第2起食物中毒事件的副溶血性弧菌属于ST199型克隆,wgSNP差异数为1~3 bp,判定为同一克隆。两次食物中毒事件患者的副溶血性弧菌间wgSNP差异数为50113~50114 bp,判定为不同克隆。7株来自患者的副溶血性弧菌携带外毒素基因tdh和tlh,来自餐具的仅携带tlh。8株副溶血性弧菌均仅携带blaCARB型耐药基因,且对氨苄西林中度敏感。结论两次食物中毒事件病原体均为副溶血性弧菌;两次食物中毒事件的副溶血性弧菌属于不同克隆,判断其无时空传播关系。未出现多重耐药现象,但均对抗生素氨苄西林中度敏感,建议临床治疗时尽量避免使用此类抗生素。Objective To implement traceability analysis and characterize the virulence genes and antibiotic resistance of Vibrio parahaemolyticus(V.parahaemolyticus)isolated from two food poisoning incidents in Chongqing.We hope that those work can provide reference for the prevention,control and clinical treatment of foodborne diseases caused by V.parahaemolyticu.Methods Automatic microbial mass spectrometer detection system was used to identify the species of pathogenic bacteria.Whole genome sequencing(WGS)was performed using Illumina sequencing platform.MLST and wgSNP were used for traceability analysis.Virulence and resistance genes were confirmed with Database alignment.Antibiotic susceptibility testing of V.parahaemolyticu was assessed using micro-broth dilution method.Results A total of 8 strains of V.parahaemolyticu were identified,including 7 strains isolated from patients and 1 from tableware.Four strains of the first food poisoning belonged to ST2156 clone,and there was no SNP were found.Therefore,they were judged as the same clone.Three strains of the second food poisoning belonged to ST199 clone,and 1to3 SNPs were found.They were also judged as the same clone.The isolates in different poisoning incidents have more than fifty thousand SNPs,which indicated they were belong to different clones.Strains from patients were carried exotoxin genes tdh and tlh,while that from tableware only carried tlh.All 8 strains carried only resistance gene blaCARB and were moderately sensitive to ampicillin.Conclusion The pathogens of the two food poisoning incidents are V.parahaemolyticu,which reminded us that we should strengthen the popular science education of bacterial foodborne diseases for reduce the occurrence of such food poisoning incidents;V.parahaemolyticu in two food poisoning incidents belonged to different clones,so it was judged that there was no transmission relationship.No multi antibiotic resistance strains were found,but they are moderately sensitive to the antibiotic ampicillin.It is recommended to avoid the us
