试析中医基本病机的内涵与实质

摘　　要：目的中医认为精、气、神统摄着人体生命活动,其中精为本,化气而舍神,精足则气壮而神明,神明则精气满而不实,疏泄得当,人得安和。该文从精、气、神的角度入手,探析其与阴阳、邪正、气血之间的联系,分层阐述疾病产生的实质与内涵。方法以中医基础理论为指导、结合中医经典与各家学说。结果疾病的基本病机不外乎阴阳失调、邪正盛衰和气血失调,其中三者存在着种属概念的上中下层次差。上位即阴阳失调和邪正盛衰对应神病和气病,为无形之病;下位概念的气血失调,对应精病,为有形之病。气机失调则在阴阳失调和气血失调之间扮演着中介与桥梁作用为中位。结论上中下位贯穿着疾病发展的始终,是疾病的基本病机。Objective Traditional Chinese medicine believes that essence,Qi and spirit govern the life activities of the human body.Among them,essence is the foundation,transforming Qi and producing spirit.Sufficient essence makes Qi strong and spiritual,and spirit is full of essence and Qi will be healthy and harmonious.This article started from the perspective of essence,Qi and spirit,and analyzed the relationship between them and Yin and Yang,evils and the healthy Qi and Qi and blood,and expounded the essence and connotation of the disease in layers.Methods It was guided by the basic theory of traditional Chinese medicine and combined with traditional Chinese medicine classics and theories of various scholars.Results The basic pathogenesis of the disease is nothing more than the imbalance of Yin and Yang,the fight of evils and healthy Qi and the imbalance of Qi and blood,among which there is a hierarchical difference in the concept of among the three.The upper level is the imbalance of Yin and Yang and the the fight of evils and healthy Qi corresponds to mental illness and Qi disease,which are invisible diseases;the lower concept of Qi and blood disorder corresponds to the essence disease,which is a visible disease.Qi movement imbalance plays the role of intermediary and bridge between Yin-Yang imbalance and Qi-blood imbalance.Conclusion The upper,middle and lower positions run through the development of the disease and are the basic pathogenesis of the disease.
