非医学院校附属医院超声医学科住院医师规范化培训工作的探索与实践

摘　　要：该文分析在非医学院校附属医院超声医学科开展住院医师规范化培训(简称住培)工作相对于医学院校附属院校及非地市级医院在住培工作中存在的问题及其产生的具体原因,并进行了探索及实践。非医学院校面临的主要问题包括招生规模不足、质量参差不齐,培训对象婚育早,指导医师教学观念、意识及能力等存在不足。问题产生的原因为地区经济发展程度,医院数量、发展规模的限制,医疗卫生领域职称政策不统一,与省属医学院校附属医院招收住院医师存在竞争且处于劣势,地级市婚育观念相对保守,非医学院校附属医院超声医学科开展系统完整的教学工作晚,指导医师知识储备及更新速度慢于学科发展速度。大同市第三人民医院和山西医科大学第一附属医院进行了探索和实践,包括落实“两个同等对待”政策进行政策保障,保障绩效奖励,通过多种方法强化住院医师动手能力,设计差异化、个体化的教学方案,培养住院医师日常工作的胜任力,通过互联网等多种教学模式提高教学质量。To analyze the existing problems and reasons in the implementation of residency training in non-medical universities affiliated hospitals in comparison with those of medical universities,and make some explorations and suggestions.The existing problems include a small enrollment scale,varying qualifications of enrolled students,early marriage and fertility,outdated teaching philosophy and low qualifications of mentors.Possible reasons behind these problems are constraints and effects of local economic development,numbers and development of hospitals,inconsistent evaluation policies of professional titles in the field of healthcare,disadvantaged position in competition with affiliated hospitals of medical universities in terms of the recruitment,prefecence for early marriage and fertility in prefectual-level cities,late start of their ultrasound departments in terms of systematic and comprehensive teaching and staff's lower speed of knowledge storage and renewal.Herein this paper suggests that firstly the policy of"two equal treatments"should be implemented,secondly various methods such as differentiated and individualized teaching should be adopted to strengthen residents'hands-on abilities and make them qualified for daily work.And different teaching modes such as online education should be adopted to enhance the quality of teaching.
