慈溪市一起新血清型副溶血性弧菌引起的食源性疾病暴发分析

摘　　要：目的 分析2021年8月慈溪市一起食源性疾病暴发中副溶血性弧菌病原学特征。方法 对患者粪便和食品标本中分离得到的菌株进行流行病学调查、细菌学鉴定、血清分型、耐药性研究、毒力基因检测和脉冲场凝胶电泳(PFGE)聚类分析。结果 分离得到9株副溶血性弧菌,5株血清分型为O10:K4,O2:K5、O2:K22、O2:KUT、O3:K54各1株;对氨苄西林均耐药,其中1株还对头孢匹美耐药;从患者中分离到的O10:K4株tdh和tlh基因阳性,trh阴性,PFGE分子分型具有较高同源性。食品中分离到的O10:K4株不携带毒力基因,PFGE分子分型带型也不一致。结论 这是一起副溶血性弧菌新血清型O10:K4混合其他血清型引起的食源性疾病暴发事件。新血清型O10:K4具有多个克隆型,在患者和环境中毒力基因表现不一致。Objective This paper aims to analyze the etiological characteristics of a V. parahaemolyticus-caused foodborne-disease outbreak in Cixi city in August, 2021. Methods Epidemiological investigation, bacteriological identification, serotyping, drug resistance analysis, virulence gene testing, and clustering analysis of pulse-field gel electrophoresis(PFGE) were performed on the strains isolated from patient stool and food samples. Results Nine strains of V. parahaemolyticus were isolated, five strains were serotyped and O10:K4, O2:K5, O2:K22, O2:KUT and O3:K54 were observed for each strain. All the strains were resistant to ampicillin, and one was also resistant to cefepime;O10:K4 strains isolated from patients were positive for tdh and tlh genes, trh negative, and showed high homology in PFGE molecular typing. The O10: K4 strain isolated in the food products did not carry the virulence genes, and the band pattern of PFGE molecular typing was also inconsistent. Conclusion This is a foodborne-disease outbreak caused by a new serotype(O10:K4) of V. parahaemolyticus mixed with other serotypes. The new serotype O10:K4 has multiple clonotypes with inconsistent virulence genes between patient and environmental samples.
