摘 要:目的 对中医院住院医师规范化培训学员进行情景模拟教学在急诊技能培训的应用进行研究。方法 选择本院进行住院医师规范化培训的学员180名,随机分为观察组与对照组各90名。对照组采用常规培训方法进行急救技能的基础知识和临床实践知识教授,包括如何识别危重患者、心肺复苏术、气管插管术、电除颤等。观察组采取情景模拟教学方法,以临床实际案例为依托,针对前述常用急救技能,选择适宜疾病和典型病例制作相关临床模拟材料,让学员主动思考,结合案例进行具体抢救方案的实施。引导学员解析病例,以及实施救治中的思维方向。每次培训将学员分成5人1组,分角色演练。设置患者心脏骤停等临床场景,住院医师进行心肺复苏和气管插管等急救操作。对教师及学员用时、学员理论及技能考核成绩、满意度、工作能力评估等方面进行评分。结果 观察组教师用时明显低于对照组,学员活跃度较对照组高(P <0.05)。两组学员的基础知识、单项技能的考核中,观察组均显著高于对照组(P <0.05)。观察组对情景模拟教学方式的技能培训满意度显著高于对照组,在基础知识培训、技能培训、课题活跃度、师生互动方面均有显著差异(P <0.05)。观察组在病情评估、诊治思维、应急能力、团队配合评分等方面均显著高于对照组(P <0.05)。结论 情景模拟教学法适用于住院医师规范化培训的技能培训,能帮助住院医师进一步巩固理论知识,学习并掌握临床知识和技能,激发其学习兴趣,提高病情评估、诊治思维、应急能力、团队配合等综合能力,教学效果好。Objective:To study the application of scenario simulation teaching in emergency skill training for standardized training of resident physicians in traditional Chinese medicine hospitals.Methods:A total of 180 students who received standardized training for resident physicians in our hospital were randomly divided into an observation group and a control group,90 students in each group.The control group was taught basic knowledge and clinical practice knowledge of emergency skills using conventional methods,including how to identify critical⁃ly ill patients,cardiopulmonary resuscitation,tracheal intubation,electric defibrillation,etc.The observation group adopted a scenario simulation teaching method,based on clinical actual cases,and selected suitable diseases and typical cases to produce relevant clinical simulation materials for the commonly used emergency skills mentioned above.Students were encouraged to actively think and implement specific rescue plans based on the cases.Stu⁃dents were guided to analyze cases and develop thinking directions in implementing treatment.Each training ses⁃sion would divide the trainees into groups of 5 people and conduct role-based exercises.Clinical scenarios such as sudden cardiac arrest for patients were set up,and resident physicians performed emergency operations such as cardiopulmonary resuscitation and tracheal intubation.Teachers and students in terms of time usage,theoretical and skill assessment scores,satisfaction,and work ability evaluation were rated.Results:Showed that the teacher′s time in the observation group was significantly lower than that in the control group,and the activity of the stu⁃dents was higher than that in the control group,with a statistically significant difference(P<0.05).In the assessment of basic knowledge and individual skills of the two groups of students,the observation group was significantly higher than the control group(P<0.05).The satisfaction of the observation group with the skill training of scenar⁃io simulation teaching m
