摘  要:目的探索以案例为基础的教学法(case-based learning,CBL)结合模拟诊疗教学法在眼科住院医师规范化培训中的应用及效果评价。方法选择2017年9月—2022年8月大连医科大学附属第二医院眼科住院医师规范化培训学员作为研究对象,共58名,随机分为对照组与观察组。对照组采用传统教学方式教学,观察组采用CBL+模拟诊疗教学方式教学。两组学员均采用理论考试+技能考试对教学效果进行评估,通过调查问卷来评估观察组学员的感受和体验。结果观察组笔试总分、基础理论成绩、病例分析成绩、技能考试成绩[(81.24±3.57)分、(40.83±2.92)分、(40.62±2.37)分、(85.28±5.19)分]均高于对照组[(75.90±4.75)分、(38.10±3.24)分、(37.79±2.77)分、(80.52±5.10)分],差异有统计学意义(P<0.05)。观察组学员认为新的教学模式“激发了对眼科的兴趣”,占比为93%(27/29);认为“有助于提高分析病历的能力,有助于提高临床技能运用能力,有助于提高与患者沟通能力”,占比分别为90%(26/29)、83%(24/29)和80%(23/29)。93%(27/29)的学员对此项教学模式满意。结论CBL结合模拟诊疗教学法可激发医学生学习热情与主观能动性,取得了良好的教学效果,综合提升眼科住培学员对常见眼病的诊疗经验。Objective To explore the application and effect evaluation of case-based learning combined with simulation diagnosis and treatment in the residents standardized training of ophthalmology.Methods A total of 58 residents from department of ophthalmology,the Second Affiliated Hospital of Dalian Medical University from September 2017 to August 2022,who participated in standardized resident training were selected for research.The residents were randomly divided into the control group and the observation group.The control group was taught with traditional teaching methods,and the observation group was taught with CBL+simulated diagnosis and treatment teaching methods.Both groups of residents evaluated the teaching effectiveness using a theory exam and a skills exam,and evaluated the feelings and experiences of the observation group through a survey questionnaire.Results The total score of the theoretical examination,basic theory score,case analysis score,and skill test score of the observation group[(81.24±3.57)points,(40.83±2.92)points,(40.62±2.37)points,and(85.28±5.19)points]were higher than those of the control group[(75.90±4.75)points,(38.10±3.24)points,(37.79±2.77)points,and(80.52±5.10)points],and the difference was statistically significant(P<0.05).The proportion of the residents in the observation group who recognized the new teaching mode as"stimulating interest in ophthalmology"was 93%(27/29),and the proportion of"helping to improve the ability to analyze medical records,improve the ability to use clinical skills,and improve the ability to communicate with patients"was 90%(26/29),83%(24/29),and 80%(23/29),respectively.93%(27/29)of the residents were satisfied with this teaching mode.Conclusion CBL combined with simulated diagnosis and treatment teaching method can stimulate the learning enthusiasm and subjective initiative of medical residents,obtain good teaching effects,and comprehensively improve the clinical experience of ophthalmic residents on common ophthalmic diseases.
