摘 要:目的:探讨模拟临床环境联合PDCA〔plan(计划)、do(实施)、check(检查)、action(处理)〕循环管理模式在心内科教学中应用效果。方法:以2021年12月开展模拟临床环境联合PDCA管理为分界线,将2021年10~11月上海长征医院46名心内科实习护士实施情景模拟教学归为对照组,将2021年12月至2022年1月46名心内科实习护士实施模拟临床环境联合PDCA管理归为研究组。结束后采用评判性思维能力测量表对教学前后护生评判性思维进行评分(CTDI-CV);比较两组护理文书书写合格率、理论、实践技能、综合素质考核和教学满意度。结果:教学后,两组评判性思维能力包括寻找真相、分析能力、评判思维的自信心、开放思想、认知成熟度、求知欲和系统化能力评分均显著升高( P<0.05),且研究组各维度评分显著高于对照组( P<0.05);教学后,研究组体温、医嘱、护理记录合格率均显著高于对照组( P<0.05);研究组理论、实践技能、综合素质考核成绩显著高于对照组( P<0.05);教学满意度显著高于对照组( P<0.05)。 结论:模拟临床环境联合PDCA循环管理可明显提升心内科护生的评判性思维和理论、实践技能及综合素质能力,提高教学满意度,值得临床推广应用。Objective To explore the application effect of simulated clinical environment combined with plan,do,check,action(PDCA)cycle management model in the teaching of cardiology.Methods Taking the implementation of simulated clinical environment combined with PDCA management in December 2021 as the dividing line,46 practice nurses in cardiology department of Shanghai Changzheng Hospital from October to November 2021 who implemented scenario simulation teaching were classified as the control group.A total of 46 practice nurses in cardiology department from December 2021 to January 2022 were included in the study group who implemented simulated clinical environment combined with PDCA management.The Critical Thinking Disposition Inventory-Chinese Version(CTDI-CV)was used to evaluate the critical thinking disposition of nursing students before and after the teaching.The qualification rate of nursing document writing,theory,practical skills,comprehensive quality assessment and teaching satisfaction were compared between the two groups.Results After teaching,critical thinking ability including truth seeking,analytical ability,critical thinking self-confidence,open mind,cognitive maturity,thirst for knowledge and systematic ability scores of two groups were significantly increased(P<0.05),and the scores of each dimension in the study group were significantly higher than those in the control group(P<0.05).After teaching,the qualified rates of body temperature,doctor's advice and nursing records in the study group were significantly higher than those in the control group(P<0.05).The scores of theory,practical skills and comprehensive quality assessment of the study group were significantly higher than those of the control group(P<0.05).The teaching satisfaction of the study group was significantly higher than that of the control group(P<0.05).Conclusions Simulated clinical environment combined with PDCA cycle management can significantly improve the critical thinking,theoretical,practical skills and comprehensive quality ability
